便携式超声在大地震应急救治中的应用

摘　　要：目的总结便携式超声在大地震前沿医院医疗救治中的应急管理及其应用。 方法整合科室人力资源，改变排班模式，优化服务流程，采用高危优先的原则，应用便携式超声对我国四川省汶川“5．12”大地震中的1471例伤员进行胸腹部脏器全面筛查，对胸腔、腹盆腔积液者尽可能在超声下定位及超声引导诊断性腹穿；阳性病例定时复查。 结果本组153例（10．4％）地震伤患者有异常超声影像表现。提示有胸腹部损伤111例（7．5％）；腹部损伤51例（3．5％），胸腔积血84例（5．7％），胸腹部复合伤24例（1．4％）。地震伤后继发性病变50例（3．4％）；尿潴留15例（1．0％），有急性肾功能衰竭超声影像改变者35例（2．4％）。其中5例（0．3％）腹部脏器损伤者经手术证实，46例（3．1％）腹部损伤经超声定期复查变化不大而进行保守治疗。 结论在大地震伤员应急救治中便携式超声有极其重要的作用。Objective To summarize front line hospital stringency administration and application of mobile ultrasound in the May 12 deadly earthquake emergency treatment. Methods To integrate human resource and chang on duty mode and optimize service flow and high risk patients first examination. 1 471 patients of earthquake injury were screened in the thorax and abdominal part. The patients of pleural effussion and seroperitoneum and pelvic cavity fluidify were localizationed and diagnostic punctured by mobile ultrasound guidance. The positive patients were cheehed oriented to time. Results 153 cases among 1 741 patients of earthquake injury had the abnormal ultrasonic appearance （10. 4%）, 111 patients were injured in the thorax and abdominal injury （7.5%）, 51 cases in abdominal injury （3.5%）, 84 cases hemothorax （5.7%）, 24 cases thorax and abdominal combined injury （1.4%） . 50 patients had the secondary change （3.4 % ）, 15 cases had retention of urine （1.0 %）, 35 cases had ultrasonic image appearance of acute kidney nonfunction （2.4%） . 5 cases abdominal organs damaged were confirmed by operation （0. 3%） . 46 patients the ultrasonic image appearance in abdominal injury had no change were expectant treated by regularity examination （3.1 % ） . Conclusions It is very important the application of mobile ultrasound in the deadly earthquake emergency treatment.
