摘　　要：目的:比较分析河南省食管癌高发区同卵双胞胎成员食管、贲门和胃窦黏膜活检组织病理结果,探讨遗传和环境因素对食管和贲门癌变的影响。方法:对108对来自于河南省食管癌高发区的同卵双胞胎进行问卷调查,对其中22对自愿接受胃镜检查者进行黏膜活检组织病理学检查。结果:①108对同卵双胞胎中,共发现癌症13例,其中食管癌7例(均为男性,单发),食管癌发生率0.065%(7/108)。②接受胃镜检查的22对双胞胎中4例食管癌患者,1例胃癌患者,癌前病变3例,均为单发;18对未患癌患者中,3对慢性轻度食管炎和1对慢性浅表性胃炎为双发,其余14对所患疾病均为单发。结论:环境因素对食管癌变的影响远大于遗传因素。Aim ： To determine the impact of genetic and environment factors on esophageal and gastric cardia carcinogenesis by comparing the alternations of esophageal and gastric cardia precancerous lesions of monozygotic twins.Methods：Questionnaire （n = 108 pairs） endoscopy biopsy and histopathological examination （n = 22 pairs） were applied. Results：Of the 108 pairs of monozygotic twins from the high-incidence area tor esophageal cancer （EC） in Henan province, 7 （0. 065% ） male cases were identified with EC （occurred only in one member of the twins）. Of the 22 monozygotic twins examined with endoscopy and biopsy,there were 4 cases with EC（occured only in one member of the twin） ,1 case with gastric adenocarcinoma,3 cases with esophageal precancerous lesions;of the 18 non-cancer twins, different epithelial lesions were identified in 14 pairs of twins and only in one member of the twin,excepting for 3 pairs of chronie supor filial gastritis and 1 pair of chronic mild esophagitis. Conclusion：Environmental factors may play a major role in esophageal carcinogenesis.
