摘 要:目的:研究胃腺癌中是否存在分泌血管活性肠肽(VIP)的肿瘤细胞,探讨VIP在胃腺癌发生中的作用。方法:通过免疫组化方法,检测30例正常胃窦黏膜,30例不典型增生胃黏膜和74例胃腺癌组织中VIP蛋白的表达,以及中分化胃腺癌细胞系SGC-7901,低分化胃腺癌细胞系BGC-823及未分化胃腺癌细胞系HGC-27中VIP蛋白的表达。结果:36/74例(48.6%)胃腺癌组织中发现VIP阳性细胞,其中高分化7例,中分化15例,低分化14例。阳性细胞形态不规则,多数为细胞浆着色,少数细胞膜着色,核大而不规则。4/30例(13.3%)不典型增生及3/30例(10.0%)正常胃窦黏膜存在VIP阳性细胞。胃腺癌细胞株VIP蛋白呈高水平表达,其中SGC-7901阳性率为71.5%,BGC-823阳性率为82-3%,HGC-27阳性率为92.8%,VIP免疫反应阳性颗粒主要出现在胞浆。结论:胃腺癌组织的肿瘤细胞有较高VIP蛋白表达;胃腺癌细胞株分泌VIP蛋白,且阳性率有随其分化程度降低而升高的趋势。推测VIP蛋白可能与胃腺癌的发生有密切的关系。Objective: To determine the role of the vasoactive intestinal peptide (VIP) protein expression in gastric adenocarcinoma tissues and gastric adenocarcinoma cell lines. Methods: We detected VIP protein in the antral mucosa of 30 healthy controls, gastric mucosa of 30 patients with dysplasia, and carcinoma tis- sues of 74 patients with gastric adenocarcinoma using immunohistochemistry. Meanwhile, SGC-7901 (moderately differentiated), BGC-823 (poorly differentiated) and HGC-27 (undifferentiated) gastric adenocarcinoma cell lines were cultured and then tested for VIP protein expression. Results: Of the 74 gastric carcinoma tissue samples, 36 (48.6%) were positively stained by anti-VIP antibody. The 36 VIP positive cases included 7 well differentiated adenocarcinoma tissue samples, 15 moderately differentiated adenocarcinoma tissue samples and 14 poorly differentiated adenocarcinoma tissue samples. Most of the positively stained particles were distributed in the cytoplasm, but there were a few noted in the cell membrane. Of the 30 gastric mucosa samples from patients with dysplasia, 4 (13.3%) cases were positive for VIP expression. Of the antral mucosa samples from the 30 healthy controls, 3 (10.0%) cases had positive VIP expression. The positive rate of VIP expression was 71.5% in SGC-7901 cells, 82.3% in BGC-823 cells, and 92.8% in HGC-27 cells. Most of the positively stained particles were distributed in the cytoplasm. Conclusion: Gastric adenocarcinoma tissues highly express VIP protein and the gastric carcinoma cell lines SGC-7901, and BGC-823, and HGC-27 secrete VIP, features that are negatively correlated with differentiation. VIP may be an integral part in the pathogenesis of gastric adenocarcinoma.
