胃癌hTERT、Syk表达与分子边界的临床研究

摘　　要：目的探讨胃癌及其切缘组织中hTERT、Syk表达及其分子边界与手术切缘的安全性关系。方法应用免疫组化方法检测39例胃癌标本及其切缘组织中的hTERT、Syk的表达。结果(1)hTERT在胃癌组织中阳性表达率为69.2%,近端切缘距癌肿4 cm的阳性表达率为0,远端切缘距癌肿3 cm的阳性率为0。(2)Syk在胃癌的阳性表达率为5.1%,肿瘤旁正常胃黏膜组织中表达率为100.0%。(3)hTERT的阳性表达从癌中心向胃癌周边切缘组织逐渐递减,而Syk则相反,在近端切缘癌中心与癌旁3 cm、4 cm处比较差异有统计学意义(P<0.05),在远端切缘癌中心与癌旁2 cm、3 cm处比较差异有统计学意义(P<0.05)。结论hTERT和Syk在胃癌及其周边组织中存在着分子边界,距胃癌中心近端4 cm及远端3 cm是hTERT、Syk的分子边界。Objective To investigate the the expressions of human telomerase reverse transcriptase（hTERT）and spleen tyrosine kinase（Syk） in the gastric cancer and its peripheral tissues and to explore the molecular margin in the gastric cancer for evaluating the security of surgical margin.Methods The expressions of hTERT and Syk were detected by immunohistochemistry in 39 cases of gastric cancer and peripheral tissues 1 cm,2 cm,3 cm,4 cm and 5 cm away from the cancer center.Results（1）The positive rate of hTERT in gastric cancer was 69.2%,the positive rates in 4 cm away in the upper end and 3 cm away in the lower end of margin tissues were both 0.（2）The positive rates of Syk in gastric cancer and normal gastric tissues were 5.1% and 100.0%,respectively.（3）The positive rate of hTERT expression from the cancer center to the surrounding tissues was gradually diminishing.Contrarily,the positive rate of Syk expressiom from the cancer center to the peripheral tissues was gradually increased.In the upper...更多 surgical margins,the rate of Syk in the 4 cm away was significantly higher than that in the 3 cm away peripheral tissues（P〈0.05）.In the lower tumor surgical margins,the rate of Syk in the 3 cm away was significantly higher than that in the 2 cm away peripheral tissues（P〈0.05）. Conclusions The molecular border exists considering the expressions of hTERT and Syk in gastric cancer and the surrounding surgical margin tissues and 4 cm away in the upper end and 3 cm away in the lower end of margin tissues can be defined as molecular margin.
