CDX2 p-p38蛋白在肠上皮化生及胃癌组织中的表达

摘　　要：目的探讨CDX2、p-p38在胃黏膜病变中的表达及其意义。方法采用免疫组织化学方法检测CDX2和p-p38在肠上皮化生和胃癌组织中的表达情况。结果CDX2在正常胃黏膜中无表达,在肠上皮化生、肠型胃癌、弥漫型胃癌组织阳性表达率分别为84.6%、57.9%、13.3%,肠型胃癌组织中CDX2水平显著低于肠上皮化生(P<0.05),肠型胃癌显著高于弥漫型胃癌(P<0.05);p-p38在正常胃黏膜中呈少量表达,在肠上皮化生、肠型胃癌、弥漫型胃癌组织阳性表达率分别为80.0%、15.8%、86.7%,肠型胃癌组织中p-p38水平显著低于肠化生(P<0.05),肠型胃癌显著低于弥漫型胃癌(P<0.05)。在肠化生中CDX2的表达与p-p38的表达呈正相关(P<0.05),而在肠型胃癌两者表达无显著相关性(P>0.05),弥漫型胃癌中两者表达也无显著相关性(P>0.05)。结论CDX2在肠上皮化生向胃癌尤其肠型胃癌发展过程中可能起到重要作用,而p-p38在肠化生形成中对CDX2表达可能起正向调节作用;p-p38在弥漫型胃癌中明显高表达可能与胃癌的发展有关。Objective To investigate the expression and significance of CDX2 and p - p38 in the gastric mucosal lesions. Methods The levels of CDX2 and p - p38 were investigated in the intestinal metaplasia （IM） and gastric carcinoma by immunohistochemistry. Results The positive expression rate of CDX2 was 84.6% in IM,57.9% in intestinal - type GC and 13.3% in diffuse - type GC,while neither CDX2 is expressed in the normal gastric mucosa. It shows there is a significant difference between the IM and intestinal - type GC （ P 〈 0. 05 ）, between the intestinal - type GC and diffuse - type GC is so（ P 〈 0.05）. The positive expression rate of p - p38 was 80. 0% in IM,15.8% in intestinal- type GC and 86.7% in diffuse- type GC ,while the expression of p- p38 is little in the normal gastric mucosa. It shows there is a significant difference between the IM and intestinal - type GC （ P 〈 0. 05）, between the diffuse - type GC and intestinal - type GC is so（ P 〈 0.05 ）. The expression of CDX2 was positively correlated with the expression of p- p38 （in IM, P 〈0.05） ,which is not significantly correlated with the expression of p - p38 （ in intestinal - type, P 〉 0.05, in diffuse - type GC, P 〉 0.05 ）. Conclusion CDX2 may play an important role in the pathogenesis of gastric carcinoma especially intestinal - type gastric carcinoma, and p - p38 may regulate the expression of CDX2 in the formation of intestinal metaplasia;the significant expression of p -p38 in the diffuse -type GC may be related to the development of the GC.
