胰头癌手术130例疗效评价和影响因素分析

摘　　要：目的探讨胰头癌行胰十二指肠切除术的疗效及其影响因素。方法对130例胰头癌行胰十二指肠切除术患者的术后并发症、死亡率、术后生存情况及影响因素进行调查。结果手术损伤肠系膜血管及门静脉3例;手术并发症45例(34.6%);手术死亡12例(9.2%)。术后1年、3年和5年存活率分别为44.6%、11.5%和2.3%;肿瘤直径、淋巴结转移和切缘状态是影响患者生存的主要因素,而癌分化程度和患者年龄对5年存活率无明显影响(P>0.05)。结论胰头癌施行胰十二指肠切除术后,并发症及死亡率高,5年存活率低,远近期效果均不十分满意。Objective To evaluate surgical treatment effect of the cancer of the head of the pancreas and ana- lyze factors that influence survival.Method We investigate the operative complications,mortality, and postoperative quality of life of the patients.Results Accidental injury of upper mesenteric vessel or portal vein occured in 3 cases.The rate of operative complication and mortality was 34.6% and 9.2%,respectively.The 1—,3—,and 5—year survival rate for 130 patients after pancreatico- duodenectomy were 44.6%,11.5% and 2.3%,respectively.The diameter of the tumor(P<0.05),the metastasis of lymph nodes (P<0.05),the status of the resection margins (P<0.01) were the influenc- ing survival factors,while the histological grade of the tumor and the age of the patients were not signif- icant influencing survival factors (P>0.05).Conclusion Pancreaticoduodenectomy for cancer of the head of the pancreas incurs major trauma,long operative time,brings about high rate of operative com- plication and mortality,and yields low 5—year survival rate.
