乳腺癌保留乳房手术疗效评估

摘　　要：目的研究乳腺癌行保留乳房手术的疗效。方法100例患者分为保留乳房根治术50例和改良根治术50例，行肿瘤广泛切除加腋淋巴结清扫，术后常规行辅助放疗、化疗及内分泌等治疗，比较两种术式疗效。结果100例乳腺癌中，生存83例（83．0％），死亡17例（17．0％），淋巴结转移30例（30．0％），局部复发13例（13．0％）；ER、PR均阳性62例（62．0％），两者均阴性18例（18．O％），仅ER阳性12例（12．O％），仅PR阳性8例（8．0％）；两组生存率、复发率差异无统计学意义（X^2=3．503，X^2=4．284，P〉0．05），两组转移率差异有统计学意义（X^2=6．789，P〈0．05）；大于12个月主观满意度99．0％，乳房美观检测数据提示，90％以上的保留乳房根治术均能获得理想的外观。结论保留乳房的乳腺癌根治术与改良根治术疗效相同，并能使患者获得满意的乳腺外观。Objective To study the curative effect of breast-conserving therapy in patients with breast cancer. Methods I00 eases with breast cancer were divided into breast-conserving therapy 50 eases and radical therapy 50 cases. The primary manner was wide excision of tumor and dissection of axillary lymph node. Adjuvant radiotherapy,chemotherapy and ineretion treatment were given postoperatively. Results In 100 cases of breast cancer,the sur- vival of 83 eases（83.0% ） ,17 patients（ 17.0% ） died,30 eases（30. 0% ） of metastasis ,local recurrence in 13 eases （ 13.0% ） ;ER,PR-positive are 62 cases （ 62.0% ）, both wholly negative in 18 cases （ 18.0% ）, ER-positive only 12 cases （ 12. 0% ）, only 8 eases （8.0%） of PR positive ; comparing two groups, survival and recurrent rate had no significant difference（ X^2 = 3. 503, X^2 = 4. 284, P 〉 0. 05 ） , metastasis rate was significant different （ X^2= 6. 789, P 〈 0. 05 ） ; satisfaction degree was 99.0% more than 12 months ,and got ideal appearance was over 90%. Conclusion Breastconserving therapy can get long-term effect like Auchincol＇ s. Breast-conservlng therapy can gain good cosmetic resuits.
