摘 要:目的研究DNA结合/分化抑制蛋白-1(Id-1)在肝细胞癌(HCC)中的表达及其临床意义。方法HCC手术切除标本40例,癌旁肝组织23例(离癌灶3 cm),并以15例正常肝组织为对照,用链酶抗生物素蛋白-过氧化物酶免疫组织化学(SP)方法检测Id-1、血管内皮生长因子(VEGF)、胰岛素样生长因子-1受体(IGF-1R)和Bcl-2的表达,并分析Id-1与其他各指标的关系。结果Id-1、VEGF、IGF-1R和Bcl-2在HCC中的阳性表达率(分别为85%、77.5%、85%、40%)及表达强度均明显高于对照组。Id-1的表达与HCC的分化程度呈正相关,即癌细胞分化越高Id-1表达越强;Id-1在转移的或>5 cm的HCC中的表达低于无转移的或<5 cm的HCC;VEGF的表达与HCC的分化程度呈正相关,而IGF-1R的表达与HCC的分化程度呈负相关;Bcl-2的表达与HCC的分化程度无关;Id-1的表达与Bcl-2无相关性,而与VEGF呈正相关(r=0.601,P<0.001),与IGF-1R呈负相关(r=-0.377,P=0.016)。结论Id-1在HCC中参与了肿瘤血管形成和细胞增殖调控,但可能不直接参与抑制细胞凋亡的调控。Objective To study the expression of inhibitor of DNA binding-1(Id-1) in relation to those of vascular endothelial growth factor(VEGF), insulin-like growth factor I receptor(IGF-1R) and Bcl-2, and the potential roles of Id-1 in carcinogenesis of HCC. Methods 40 cases of HCC specimens and 23 cases of adjacent non-tumor liver tissue specimens were obtained from 40 patients with different cell differentiation undergoing hepatectomy for HCC, and 15 eases of normal liver tissue specimens were used for comparison. Immunohistochemical technique was applied to detect the expressions of Id-1, VEGF, IGF- 1R and Bcl-2. Results The positive rates of Id-1, VEGF, IGF-1R and Bcl-2 in tumor tissues, which were 85%, 77.5%, 85% and 40% respectively, as well as their intensity, were significantly higher than those in normal liver tissues. Id-1 expression was positively correlated with the differentiation of HCC, and the positive rate of Id-1 was significantly correlated with the tumor metastasis and tumor size. VEGF expression was positively correlated with the differentiation of HCC, while IGF-1R expression was negatively and Bcl-2 was not correlated with the differentiation of HCC. The expression of Id-1 was positively associated with VEGF, negatively with IGF-1R, and not with Bcl-2. Conclusion Id-1 may be involved in angiogenesis of HCC and the regulation of HCC cell proliferation, but not directly in the regulation of apoptosis.
