摘 要:目的探讨食管鳞癌中Survivin、Bcl-2蛋白的表达及其与细胞凋亡之间的关系。方法应用免疫组织化学S-P法检测39例食管鳞癌组织、23例不典型增生组织及39例正常食管黏膜组织中Survivin、Bcl-2表达情况,并用TUNEL法原位检测细胞凋亡情况。结果Survivin在癌组织中表达率为59.00%(23/39),癌旁不典型增生组织和正常食管黏膜组织中未见表达。Survivin的表达与肿瘤的淋巴转移关系密切,有淋巴转移组Survivin阳性率明显高于无淋巴转移组(P<0.05)。Bcl-2在癌组织、癌旁不典型增生组织中的表达率为69.20%(27/39)、21.7%(5/23),而正常食管黏膜组织中未见表达。Bcl-2的表达与肿瘤组织的分化程度、淋巴结转移有关,有淋巴转移组Bcl-2表达阳性率明显高于无淋巴转移组(P<0.05)。在食管鳞癌组织中Survivin与Bcl-2的表达显著相关(P<0.01)。凋亡指数(AI)在癌组织、癌旁不典型增生组织和正常食管黏膜组织中分别为(4.48±0.69)、(12.67±2.20)和(30.13±4.21),组间比较差异有显著性(P<0.01)。Survivin阳性表达组及Bcl-2阳性表达组的AI明显低于其阴性表达组AI(P<0.01)。结论Survivin和Bcl-2阳性表达与食管鳞癌的生长及转移有关,两者可能通过共同抑制细胞凋亡,参与食管癌的发生发展。[Objective] To examine the expression of Survivin and Bcl-2 protein in esophageal carcinoma (EC) as well as their relationship with apoptosis. [Methods] The expression of Survivin and Bel-2 was detected by immunohistoehcmical (SP) methods in 39 cases of EC tissue, 23 cases of mucosa adjacent to cancer and 39 cases of nomal esophageal mucosa. Cell apoptosis was examined by TUNEL method. [Results] The positive rate of Survivin protein expression was 59.0% (23/39) in cancer tissue, none in mucosa adjacent and nomal mucosa tissue. The expression of Survivin was related to lymph node metastasis (LNM). The positive rate of Survivin expression in LNM eases was significandy higher than that in eases without LNM (P 〈0.05). The positive rate of Bcl-2 protein expression was 69.2% (27/39) in cancer tissue, 21.7% (5/23) in mucosa adjacent to cancer, but none in nomal mucosa tissue. The positive rate of Bcl-2 expression in LNM cases was significantly higher than that in cases without LNM (P 〈0.05). There was a close relation between the expression of Survivin and Bcl-2 protein in the carcinoma tissues (P 〈0.01). The apoptosis index (AI) was (4.48±0.69) in carcinoma tissues, (12.67±2.20) in mucosa adjacent to cancer and (30.13±4.21) in nomal mucosa tissue, with a significantly lower than their negative cases (P 〈0.01). [Conclusions] The high expression of Survivin and Bcl-2 protein is correlated with carcinogenesis, progression and metastasis of EC. Survivin and Bcl-2 protein maybe contribute to the occurrence of EC through cell apoptosis inhibition.
