炎症性肠病患者红细胞指数和血红蛋白的变化及其与疾病活动性的关系

摘　　要：目的评价炎症性肠病患者及正常对照者之间红细胞指数和血红蛋白的差异及其与疾病活动性的关系。方法队列研究271例炎症性肠病患者和正常对照者的红细胞指数和血红蛋白,分析红细胞指数和血红蛋白的改变和炎症活动性的关系。结果克罗恩病患者血红蛋白浓度(P<0.01)、平均红细胞容积(P<0.05)、平均红细胞血红蛋白含量(P<0.01)和平均红细胞血红蛋白浓度(P<0.01)显著低于正常对照组;溃疡性结肠炎患者血红蛋白浓度和平均红细胞血红蛋白浓度显著低于正常对照组(P<0.01)。相关分析表明,克罗恩病患者的血红蛋白浓度和克罗恩病活动指数(rs=-0.364,P=0.001)及红细胞沉降率(rs=-0.360,P=0.003)呈显著负相关;溃疡性结肠炎患者的血红蛋白浓度和红细胞沉降率呈显著负相关(rs=-0.565,P<0.001)。结论炎症性肠病患者的红细胞指数和血红蛋白发生变化,其中血红蛋白浓度的变化和疾病的活动性呈负性相关。Objective The objectives of this study were to evaluate altered erythrocyte indexes and hemoglobin （HGB） levels in patients with inflammatory bowel disease（IBD） and healthy controls, and alalyse the relationship between erythrocyte indexes,HGB levels and disease activity. Methods A cohort of 271 consecutive patients with active IBD were compared with healthy controls for erythrocyte indexes and HGB levels. Analysis were done to elucidate the relationships between altered erytbrocyte indexes and HGB levels and inflammatory parameters. Results The mean levels of HGB （ P 〈 0. 01 ） , mean corpuscular volume （ MCV ） （ P 〈 0. 05 ） , mean corpusular hemoglobin （ MCH ） （ P 〈 0. 01 ） and mean corpuscular hemoglobin cooncentration （MCHC） （ P 〈 0. 01 ）were significantly lower in patients with Crohn＇ s disease than those in healthy controls. The mean levels of HGB and MCHC were significantly lower in patients with ulcerative colitis than those in healthy controls（ P 〈0. 01 ）. Fur-thermore, HGB showed a significantly negative correlation with Crohn＇ s Disease activity index（CDAI） （ rs = -0. 364, P = 0. 001 ） and erythrocyte sedimentation rate（ESR） （ rs = -0. 360, P = 0. 003）in patients with Crohn＇ s disease;HGB also showed a significantly negative correlation with ESR in patients with ulcerative colitis（ rs= -0. 565, P 〈 0. 001 ）. Conclusion It is suggested that erythrocyted indexes and HGB alter in patients with IBD. Ahrered HGB level is negatively associated with disease activity.
