血小板参数、C反应蛋白对炎症性肠病患者活动性的评价

摘　　要：目的探讨血小板参数、C反应蛋白(CRP)对炎症性肠病活动性的评价。方法分析75例活动性和26例缓解期炎症性肠病患者的血小板参数、CRP的水平及其间的关系,比较临床严重程度对血小板参数及CRP的影响。结果活动性炎症性肠病患者血小板计数(PLT)和CRP水平明显高于缓解期患者,平均血小板体积(MPV)明显低于缓解期患者,两组差异有显著性(P<0.05);重度炎症性肠病患者PLT和CRP水平明显高于中度炎症性肠病患者,MPV明显低于中度炎症性肠病患者(P<0.05);中度炎症性肠病患者PLT和CRP水平高于轻度患者,MPV水平低于轻度患者(P<0.05)。活动性炎症性肠病患者严重程度分别与PLT和CRP之间呈正相关(r=-0.83,P<0.05;r=0.89,P<0.05),与MPV水平呈负相关(r=-0.38,P<0.05)。结论血小板参数和CRP水平能反映活动性炎症性肠病患者的临床严重程度。Objective To evaluate the role of platelet parameters and C-reactive protein（CRP） in the severity of active inflammatory bowel disease. Methods A retrospective analysis was performed in 75 patients with active inflammatory bowel disease（IBD） , and in 26 patients with IBD in remission. To estimate the correlation of the severity of active inflammatory bowel disease with platelet parameters and C-reactive protein. Results Platelet count（PLT） and CRP were found to be signifieatly higher in sera of patients with active inflammatory bowel desease than in patients in remission（ P 〈 0. 05 ）. Mean platelet volume（MPV） was significatly lower in sera of patients with active inflammatory bowel disease than in patients in remission（ P 〈0. 05）. The sera levels of PLT and CRP in inflammatory bowel disease at severe stage were significatly higher than those at medium stage （ P 〈 0. 05 ） ,MPV was signifieatly lower in sera of patients with active inflammatory bowel disease than in patients in remission（ P 〈 0. 05）. PLT and CRP were observed to be increased in patients at mild stage （ P 〈 0.05 ） , MPV was lower in patients at mild stage than light stage（ P 〈 0. 05 ）. A strong positive correlation existed between PLT and CRP level and the severity of active inflammatory bowel disease（ r = -0. 83, P 〈0. 05; r =0. 89, P 〈0. 05）. MPV was negative correlation（ r =0. 38, P 〈0. 05） . Conclusion The level of platelet parameters and CRP can reflect the severity of aetive inflammatory bowel disease.
