摘  要:目的:探讨鼻咽癌(NPC)组织VEGF-C和VEGFR-3的表达,及其和淋巴管密度(LVD)与临床病理参数的关系。方法:采用免疫组织化学PV-9000二步法检测55例NPC患者放疗前活检组织石蜡标本中VEGF-C和VEGFR-3的表达及LVD,分析其与临床病理参数之间的关系。结果:NPC无淋巴结转移组VEGF-C阳性率(61.5%)低于淋巴结转移组(89.7%),P=0.01;无淋巴结转移组VEGFR-3阳性率(73.0%)低于淋巴结转移组(89.7%),P=0.011;无淋巴结转移组LVD(15.2±5.2)低于淋巴结转移组(19.3±6.6),P=0.013。VEGF-C和VEGFR-3的表达与临床分期有关,P<0.001;而VEGF-C和VEGFR-3的表达及LVD与年龄、性别和原发肿瘤T分期无关,P>0.05。VEGF-C和VEG-FR-3均与LVD呈正相关,r值分别为0.491和0.467,P值均为0.000。结论:VEGF-C可促进淋巴管生成和NPC颈淋巴结转移。针对VEGF-C/VEGFR-3信号传导系统的抗淋巴管生成的治疗,有望成为抗NPC淋巴结转移的有效途径。OBJECTIVE: To detect the expressions of VEGF-C and VEGFR-3, and LVD in biopsy specimens of nasopharyngeal carcinoma (NPC) before radiotherapy and investigate their associations with clinico- pathologic characteristics. METHODS: The expressions of VEGF-C and VEGFR-3, and LVD in paraffin embedded specimens from 55 cases of NPC were studied with PV 9000 immunohistoehemical staining, as well as their relations to elinicopathologie characteristics were analyzed. RE- SULTS: The positive rates of VEGF-C were 61.5% and 89. 7% in the node negative group and node positive group respectively, and there was a significant difference between them (P = 0.01 ). The positive rates of VEGFR-3 were 73.0~//00 and 89. 7% respectively in the two groups, and there was a significant difference between them (P ~ 0. 011 ). The mean LVD in the node negative group(15.2±5.2) was lower than it (19.3± 6.6) in the node positive group(P=0. 013). The expressions of VEGF-C and VEGFR-3 were significantly lower in the stageⅠ,Ⅱgroup than those in the stage Ⅲ, Ⅳ group (P〈0. 001), whereas the expressions of VEGF-C and VEGFR-3 and LVD were not related to age, sex or clinical T stages (P〉0.05). With the increase of the expressions of VEGF-C and VEG- FR3, LVD increased and there were significant differences among the dif- ferent VEGF-C or VEGFR-3 groups. Positive correlations were also ob- served between the expression of VEGF-C or VEGFR-3 and LVD by Spearman correlation analysis (rs = 0. 491, P = 0. 000; rs= 0. 467, P =0. 000). CONCLUSION: The binding of VEGF-C and its specfie receptor, VEGFR-3 could promote lymphangiogenesis and node metastasis in NPC. Chin J Cancer Prev Treat ,2008,15(22):1724--1727
