支气管肺泡灌洗液中survivin基因检测在肺癌诊断及评估化疗疗效和预后中的意义

摘　　要：目的:探讨支气管肺泡灌洗液(BALF)中survivin基因检测在肺癌尤其是周围型肺癌诊断中的价值,以及评估survivin在判断肺癌化疗疗效、预后中的意义。方法:应用逆转录-聚合酶链反应(RT-PCR),检测84例肺癌患者及36例肺良性疾病患者BALF中survivin表达的情况,随访2年分析survivin基因表达与肺癌患者生存期的关系。对其中67例Ⅲa～Ⅳ期非小细胞肺癌患者化疗2周期后进行疗效评价,分析其与survivin基因表达的关系。结果:肺癌及肺良性疾病BALF中survivin基因表达率分别是54.8%(46/84)和2.8%(1/36)。周围型肺癌BALF中survivin表达率40.0%,明显高于灌洗液细胞学阳性率。BALF中survivin基因表达阴性肺癌患者,其化疗疗效明显优于survivin表达阳性者。肺癌患者BALF中survivin基因表达阴性者,2年生存率明显高于survivin基因表达阳性者。结论:BALF中survivin基因表达的检测可能对肺癌诊断具有一定价值,尤其对于周围型肺癌,其价值优于BALF中细胞学检查。survivin基因表达阳性提示可能化疗疗效差,2年生存率低。Objective To investigate the correlations between the expression of survivin mRNA in the bronchoalveolar lavage liquid （BALF） and the diagnose, prognosis and chemosensitivity of lung cancer. Methods The BALF of 84 patients with lung cancer and 36 patients with non-cancerous pulmonary disease were collected, and used for survivin mRNA detection by RT-PCR. The cytology assay was also carried out on BALF at the same time. Results The positive level of survivin mRNA was higher than that of BALF cytology assay in lung cancer group. In peripheral- type lung cancer group, the positive level of survivin in BALF was 40.0% higher than that of BALF cytology （18.0%）. Significant difference of survivin mRNA expression in patients with lung cancer was found in different TNM stages and in different prognosises and chemosensitivities. Conclusions （1） The detection survivin mRNA expression in BALF may be helpful in the early diagnosis of peripheral lung cancer. （2） Over-expression of the survivin in the lung cancer has a correlation with their TNM stages, prognosid and chemisensitivity in patiens with lung cancer.
