摘  要:目的探讨单相抑郁症患者临床症状与特定脑区功能异常的关系。方法运用情绪面孔匹配任务和功能磁共振成像技术对12名严格入组的单相抑郁症女性患者的脑功能进行研究,结合临床汉密尔顿抑郁量表24项探讨脑功能改变和临床抑郁症状的关系。结果在情绪面孔匹配任务的悲伤刺激条件下,抑郁患者双侧额下回眶部脑区激活水平与汉密尔顿抑郁量表的总得分、认知障碍因子单项得分呈显著负相关,右额下回眶部脑区激活水平与睡眠因子单项得分呈显著负相关,双侧颞下回激活水平与躯体化因子单项得分呈显著负相关。结论单相抑郁症患者的临床症状,特别是目前不能解释的多样躯体症状可能与患者的特定脑区功能异常有关。Objective To investigate the correlation between brain activation and the clinical symptoms evaluated by Hamilton depression rating scale(HAMD 24 items) in unipolar depression patients. Methods Using emotional face matching task and fMRI, brain function of 12 female unipolar depressed patients was studied. They were also rated on the Hamilton depression rating scale. The correlation between brain function change and HAMD scores was studied. Results In sad condition of the emotional face matching task, brain function change in four regions were found significantly correlated with HAMD scores negatively: Inferior Frontal Orbital Cortex(L) vs. score of cognition impediment factor; hfferior Frontal Orbital Cortex(R) vs. score of cognition impediment factor and score of sleeping factor; Temporal Inferior cortex(L and R) vs. score of somatic symptom factor. Conclusion Unipolar depression patients' clinical cognition impediment, especially their unexplained various somatic symptoms may relate with function change in certain brain areas.
