3种微型角膜板层刀对LASIK术角膜瓣并发症的影响

摘　　要：目的观察3种微型角膜板层刀头在准分子激光原位角膜磨镶术(LASIK)中对角膜瓣并发症的影响。方法采用Moria M2(旋转式微型角膜板层刀)130型、110型、90型对2 416眼行LASIK,对与角膜瓣有关并发症进行分析。结果LASIK角膜瓣并发症主要是瓣制作不全、瓣厚薄不均、游离、皱褶、移位、上皮植入、角膜Haze、圆锥角膜等,3组病例术后角膜瓣并发症的发生率为130型组622眼,角膜瓣并发症发生率为0.32%;110型组860眼,角膜瓣并发症发生率为0.47%;90型组934眼,角膜瓣并发症发生率为5.46%。结论130、110、90型均具有安全性,90型角膜瓣并发症发生率高于110、130型,在患者条件允许的情况下应首选110型,既能保留足够的角膜厚度又能降低角膜瓣并发症的发生。Objective To investigate the effects of there kinds of microkeratome on complications of corneal flap in laser in situ keratomileusis （LASIK）. Methods Using Moria M2 type 130,type 110,and type 90 in LASIK on 2 416 eyes,the complications related to corneal flaps were analyzed. Resutts The main complications included incomplete flap, uneven flap in thickness,dissociation, wrinkle, position shift, epithelial tissue planting, corneal haze, circular cone flaps, etc. The rate of complications after operation in three groups：0. 32%（622 eyes） in M2 130 group,0. 47% （860 eyes） in M2 110 group,5.46 %（934 eyes） in M2 90 group. Conclusion M2 130,M2 110 and M2 90 are all safe enough,but the complications in M2 90 are higher than those in M2 130 and M2 90. M2 110 should be the first choice if permitted by the patient＇s condition. It can both keep the enough thickness of the corneal flap and reduce the complications of the flap.
