水蛭素合用阿司匹林与氯吡格雷治疗TIA的疗效和卫生经济学比较

摘　　要：目的比较水蛭素合用阿司匹林与氯吡格雷治疗短暂性脑缺血发作(TIA)的疗效和卫生经济学差异。方法59例TIA随机分成两组,实验组口服水蛭素0.32 g/次,3次/d,阿司匹林100 mg/d;对照组口服氯吡格雷75mg/d,观察治疗后90 d内的治疗效果,比较两组不同时期内缺血性脑血管事件的病例数、出血等不良事件的发生率和治疗费用。结果两组在降低90 d内缺血性脑血管事件方面无显著性差异。水蛭素合用阿司匹林组的治疗费用为单用氯吡格雷的42%。两组出血发生率无显著性差异。结论水蛭素合用阿司匹林降低TIA患者90 d内缺血性脑血管事件风险的疗效与单用氯吡格雷相似,而治疗费用显著低于氯吡格雷,但可能会增加出血的风险。Objective To Compare the therapeutic effect and hygienic economics between hirudin with aspirin and clopidogrel for the treatment of transient ischemic attack. Methods 59 patients with TIA were randomly divided into 2 groups ： group A took aspirin 100 mg/d and hirudin 960 mg/d, group B took clopidogrel 75mg/d, every one was followed up for 90 days. The incidence of cerebral infarction and TIA recurrence were compared between the two groups, bleeding rate and cost were also compared. Results No significant difference of incidence of cerebral infarction and TIA recurrence was found between the two groups. The cost in group A was only 42 percent of that in group B. No significant difference of bleeding rate was found, either. Conclusion The therapeutic effectiveness of hirudin with aspirin was the same as clopidogrel in reducing the risk of TIA recurrence and the incidence of cerebral infarction within 90 days, although the cost was much lower than later, and risk of bleeding may be increased.
