摘 要:目的探讨钢板加异体骨板固定治疗脆骨病骨折及骨畸形的可行性及临床疗效。方法对6例脆骨病骨折及骨畸形患者中骨折者直接行钢板加异体骨板治疗,股骨或胫骨弯曲畸形者行截骨矫形术后再行骨折内固定。结果术后切口均一期愈合,无明显排异反应,未发现神经血管损伤异常表现。6例均获随访,时间1~3(2.1±0.2)年,骨折愈合时间为3~6(4.8±0.3)个月,所有肢体内固定处未再发生骨折、骨畸形愈合及骨不连等并发症,无内固定松动及螺钉断裂。4侧畸形肢体外观明显改善,2例出现其它部位的骨折。结论钢板加异体骨板治疗脆骨病骨折及骨畸形,能有效矫正肢体畸形,促进骨折愈合,固定牢靠,明显降低再次骨折及骨畸形愈合的发生率,是一种较好的治疗方法。Objective To explore the surgical feasibility and clinical outcome of treatment of fragilitas ossium fracture and deformity by using internal fixator plus allogenous bone plate. Methods 6 cases suffered with fragilitas ossium fracture and deformity were included in this study. Cases with fracture or deformity that had performed osteotomy correction of deformity were treated by using internal fixator plus allogenous bone plate. Results All wounds were cured primarily without obvious rejection. No severe complications such as injuries of vertebral artery or nerve were observed postoperatively. All cases were followed up from 1 to 3 years, averaged 2. 1 ± 0. 2 years. The average time of bone union was 4.8 ± 0. 3 months (from 3 - 6 months). No refracture, nonunion, malunion happened and no loosening and breakage of plate or screw was found. The appearance of deformity in 4 cases was corrected generally, but fracture happened in another site in two cases. Conclusions Treatment of fragilitas ossium fracture and deformity by using internal fixator plus allogenous bone plate can correct deformity in the lower limb perfectly, accelerate bone union, and decrease the possibility of refracture and malunion, which is an effective method.
