晶状体乳化术治疗闭角型青光眼并白内障

摘　　要：目的:探讨闭角型青光眼并白内障行晶状体乳化术治疗的临床疗效。方法:2005年8月—2006年10月诊治闭角型青光眼26例(31眼),其中25眼为原发性急性闭角型青光眼,6眼为原发性慢性闭角型青光眼,均有不同程度的晶状体混浊,行巩膜隧道切口晶状体乳化、折叠式人工晶状体植入术。随访1～14个月(平均10.2个月)。结果:术后平均眼压(12.20±3.40)mmHg比术前用药后平均眼压(19.50±4.26)mmHg明显降低,两者差异有统计学意义(P<0.05),中央前房深度由术前的(1.37±0.62)mm增加到术后(2.55±0.54)mm,两者差异有统计学意义(P<0.05),术后最佳矫正视力均有不同程度的提高。结论:晶状体超声乳化吸除并人工晶状体植入术,可有效治疗合并白内障的闭角型青光眼。Objective： To investigate the clinical effects of phacoemulsification in the management of angle-closure glaucoma with cataract. Methods：Among the 26 patients （31 eyes） with angle-closure glaucoma and cataract was studied, which were performed the Phacoemulsification with foldable intraocular lens implantation through scleral tunnel incision, including 25 eyes with primary acute angle-closure glaucoma, 6 eyes with primary chronic angle-closure glaucoma. The follow-up period was 1 14 months. Results： The intraocular pressure was reduced from preoperative mean of （19. 50 ± 4. 26）mmHg to postoperative mean of （11.25 ±2.50）mmHg （P〈0.05）. The mean anterior chamber depth was （1.47± 0.23）mm preoperatively and （2.35 ±0.34）ram postoperatively （P〈0.05）. The best corrected visual acuity was improved. Conclusion： Phacoemulsification and posterior chamber intraocular lens（IOL） implantation is a effective way in treating angle-closure glaucoma with cataract.
