螺旋CT对胰腺癌腹主动脉旁淋巴结转移的评价

摘　　要：目的探讨胰腺癌腹主动脉旁淋巴结转移的螺旋CT特征,为临床进行预后分析和合理地制订治疗方案提供依据。方法回顾性分析经手术病理证实的34例胰腺癌患者的螺旋CT资料,并与术后病理结果进行对照分析。结果胰腺癌腹主动脉旁转移淋巴结的直径明显大于非转移性淋巴结的直径,出现中心坏死的概率明显大于非转移性淋巴结。结论腹主动脉旁淋巴结长径>11mm,中心坏死及边缘不规则提示转移性淋巴结。Objective To investigate the main characteristics of spiral computer tomography for paraaortic lymph nodes metastases in pancreatic carcinoma and to provide the evidence for clincians to make appropriate treatment strategies and prognosis analysis. Methods CT scans and pathological results from 34 patients who had undergone regional lymphaden - ectomy for pancreatic carcinoma were reviewed. For evaluating the paraaortic lymph nodes, long - and short - axis diameters of lymph nodes were measured and axial ratios were calculated （short - axis/long - axis diameter）. Results The long axis diameter of the metastatic lymph nodes was significantly larger than that of non - metastatic ones. The short axis diameter of the metastatic lymph nodes was significantly larger than that of non - metastatic ones. The incidence rate of central necrosis of the metastatic lymph wodes was higher than that of non - metastatic ones with statistical significance. Conclusion A long axis - diameter （ 〉 11 mm） and presence of central necrosis are the suggestive morphologic features of metastatic paraaortic nodes.
