定量分析肝脏局限性低回声小病灶的超声造影结果并与增强CT诊断的对比研究

摘　　要：目的对比超声造影（CEUS）和增强CT显示呈肝局限性低回声小病灶（LSFLLs）（直径≤3cm）血流灌注模式，旨在探讨超声造影在LSFLLs良、恶性鉴别诊断中的应用价值。 方法回顾性分析42例（58个病灶）有病理结果的LSFLLs的超声造影灌注模式，对实时超声造影图像运用时间-强度曲线进行分析，并与64排螺旋CT增强检查结果对照。 结果LSFLLs诊断中，超声造影的检出率为93．10％，增强CT的检出率为87．93％，超声造影的准确性为92．59％，增强CT的准确性为86．27％。 结论CEUS对LSFLLs检出率优于或不低于增强CT，可有效动态地评价肝肿瘤的血管化程度，尤其对于CT扫描未显示的小病灶进行定性、定量诊断，并可进一步对血流灌注特点进行分析，超声造影对小肝癌的早期诊断具有更重要的临床意义。Objective To compare the efficacy of real-time contrast enhanced uhrasonography （CEUS） for mode of low echo-level small focal liver lesions （LSFLLs） with contrast enhanced helical CT. Methods Contrast Tuned Imaging Real time SonoVue enhanced ultrasonography and contrast enhanced 64 multislice scan CT were per formed on 42 patients with LSFLLs that have definite pathologic diagnosis. The imaging of CEUS were analaysised by the time intensity curve using computer assisted sofeware. Results The detection rate of low echo level small focal liv er lesions was 93.10% （54/ 58） for enhanced ultrasonography and 87.93% （51/58） for enhanced 64 multislice scan CT （P〉0.05） .The overall accuracy was 92.59% （53/56） for enhanced Ultrasonogrphy and 86.27% （50/54） for enhanced 64 multisliee scan CT （P〉0.05）. Conclusions CEUS was superior to contrast-enhanced 64 CT in finding LSFLLs, at least can comparable with CT in detection and characterization of LSFLLs. CEUS could evaluate the vas cularization of liver tumors effectively and dynamically by quantitation and qualitation, then provide more information in early diagnosis of liver carcinoma. Especially in LSFLLs that CT can not find and without vivid characterization, CEUS was superior to contrast enhanced 64 CT.
