81例胃粘膜异型增生虚实关联证标本辨治的临床研究

摘　　要：目的探讨胃粘膜异型增生虚实关联证型分布特征、癌变趋向、补虚泻实、标本辨治的临床疗效。方法采用建立的《Dys虚实关联证候辨识数据库》对Dys临床症状信息资料进行虚实关联证候辨识,并对81例Dys属虚实关联证侯不同证候类型确立补虚泻实冶法,用协定处方进行临床证候干预。结果81例虚实关联证经辨识,病例数排前5位的证候类型及其协定方有效率为:肝胃气滞并/兼气阴两虚占18.52%,有效率93.33%;湿热蕴胃并/兼气阴两虚占16.05%,有效率84.61%;寒热错杂占13.58%,有效率81.82%;肝胃气滞并/兼脾气虚弱占12.35%,有效率90%;瘀阻胃络并/兼气阴两虚占9.88%,有效率87.5%。结论肝胃气滞并/兼气阴两虚及肝胃气滞并/兼脾气虚弱治疗有效率较高。表明其发病以气滞为先导,即先滞肝胃气机,病情轻疗效好;继凝血入胃络,进入瘀凝胃络阶段,则病情重疗效较差。Objective To discuss distribution character of combination of excess and deficiency, cancerization tendency, chnical effect of tonffying deficiency and reducing excess as well as manifestation and root treatment. Methods Data- base of Dys excess and deficiency combination was used to identify combination of excess and deficiency syndrome with Dys clinical symptom information. 81 patients were treated with tonifying deficiency and reducing excess method and clinical cipher prescription. Results Among 81 cases, there were 18.52 % liver and stomach Qi stagnation ＆ Qi and Yin deficiency, with 93.33 % effective rate ; 16.05 % damp - heat accumulating in stomach ＆ Qi and Yin deficiency, 84.61% effective rate; 13.58 % complex of cold and heat, 81.82 % effective rate; 12.35 % hver and stomach Qi stagnation ＆ spleen and stomach deficiency, 90 % effective rate; 9.88 % blood stasis blocking stomach collateral ＆ Qi and Yin deficiency, 87.5 % effective rate. Conclusion Effective rate was higher in hver and stomach Qi stagnation ＆ Qi and Yin deficiency and liver and stomach Qi stagnation ＆ spleen and stomach deficiency, which indicates： Qi stagnation, mild condition happened at early stage with good result, and then developed into the severe stage of blood stasis blocking stomach collateral with less good result.
