摘  要:目的:探讨c-kit蛋白在鼻咽癌中的表达情况及其临床意义,评价伊马替尼(Imatinib)应用于鼻咽癌的可能性。方法:将石蜡包埋的153例鼻咽癌和165例正常鼻咽上皮组织制成组织芯片染色。在153例鼻咽癌患者中,42例有完整的临床资料,包括性别、年龄、分期和生存数据。结果:鼻咽癌c-kit阳性表达率为37.3%(57/153),正常鼻咽上皮阳性率为4.8%(8/165),两者差异显著(P=0.000)。42例有完整临床资料的鼻咽癌患者中,c-kit表达与性别、年龄、分期无关,c-kit阳性表达较其阴性表达的T分期更晚,但未见统计学差异(P=0.067);c-kit阴性表达的5年局域无复发率较其阳性表达高(P=0.041)。结论:c-kit蛋白的过表达更常见于鼻咽癌组织而少见于正常鼻咽上皮,c-kit过表达者5年局域无复发率低,这一结果为伊马替尼应用于鼻咽癌的治疗提供了初步的依据。Objective: c-kit protein plays an important role in carcinogenesis, progression of tumors, and it is a target of imatinib. The study was to investigate the expression and clinical significance of c-kit in nasopharyngeal carcinoma(NPC) and evaluate the possibility of treatment with imatinib. Methods : Formalin-fixed and paraffin-embedded specimens of 153 nasopharyngeal carcinomas and 165 normal epithelium of nasopharyngeal cavity were stained by tissue microarray technique. Among 153 nasopharyngeal carcinoma patients, 42 had the clinical data, including gender, age, stage and survival datas. Results:The results showed that 37.3% (57/ 153) of NPC biopsy samples were positive for c-kit, while only 4. 8% (8/165) of normal nasopharyngeal epithelium samples stained positively (P = 0. 000). c-kit expression did not correlate with gender, age, clinical stage, yet correlated with poor local regional failure free survival ( P 〈 0. 05 ), and approximately correlated with advanced T classification( P = 0. 067 ). Conclusion : Our results suggest that c-kit overexpression may be found significantly more frequently in NPC than in normal nasopharyngeal cavity tissue, and it correlates with poor local regional failure free survival. Moreover, those results provide a foundation to investigate imatinib therapy in NPC patients.
