科室是否收治地震伤员对医护人员心理状况的影响

摘　　要：目的比较并分析汶川大地震期间科室是否收治地震伤员对医护人员心理状况的影响，为心理干预提供参考和依据。方法采用方便抽样的方法，抽取收治大量地震伤员的骨科医护人员97名（骨科组）和未收治地震伤员的外科其他科室医护人员100名（未收治组），采用症状自评量表（SCL-90）进行调查。结果骨科组地震期间躯体化、焦虑、恐怖等因子得分高于全国常模，且差异具有统计学意义（P〈0．05），但阳性项目数与全国常模比较差异无统计学意义；未收治组与全国常模比较，除焦虑、恐怖两因子无显著性差异外，其他因子均低于全国常模，部分人在个别因子上出现阳性症状。结论地震期间收治地震伤员科室的医护人员存在心理伤害，需要进行心理干预；未收治地震伤员科室的医护人员心理状况总体良好，仅部分人员需要接受心理指导。Objective To compare and analyze the mental status of medical staffs between orthopedics department treating the patients injured in the earthquake and other departments which did not treat patients injured in the earthquake, so as to provide the evidence and reference for psychological intervention. Methods A cross-sectional study was done. We chose the staffs in the orthopedics department （the orthopedics group, 97 persons） and in other departments which did not treat patients injured in the earthquake （the non-treatment group, 100 persons） as the eligible subjects, by a convenient sampling. The research tool to make this survey was Symptom Checklist-90 （SCL-90）. Results The scores in somatization, anxiety and phobic anxiety of staffs in the orthopedics department were significantly higher than those in the national norm, having a significance in statistics （P 〈 0. 05 ）. But the positive numbers had no significant difference compared to the national norm. There was no significant difference in the anxiety and phobic anxiety score between departments which did not treat patients injured in the earthquake and the national norm. As for others they were significantly lower than the national norm （P 〈 0. 05 ）. Only a few persons had positive symptome in specific factors. Conclusions There exists damage in mental health in the medical staffs in orthopedics department, and it is necessary to implement mental intervention. The psychological profile of medical staffs of other departments which did not treat patients injured in the earthquake is totally fit, and only a few individuals need psychological directions.
