术前冲击化疗在N_2～N_3颈部转移癌中的应用

摘　　要：为探讨既保留重要血管又彻底切除病灶治疗 N_2～N_3的颈部转移癌,应用术前冲击化疗(DDP+5Fu)+联合根治术治愈6例喉、下咽癌颈转移患者。结果显示,冲击化疗可缩小瘤体,改善局部症状,降低手术难度,可使已固定的颈转移癌变为能活动,得以彻底切除;并不干扰手术,能提高手术切除率。与前期单纯手术组相比手术切除率有显著性差异(P<0.05)。Six patients with laryngeal and hypopharyngeal carcinoma,together with N_2～N_3 cervical metastasis,were treated by preoperative impact chemotherapy (DDP+5-Fu) and combined radical surgery.Our results confirmed that the impact chemotherapy could reduce the size of tumour,improve local symptoms,and make operation less dif- ficult.After impact chemotherapy,the unresectable cervical metastatic lymph node be- came resectable and could be resected completely.There was significant difference(P <0.05) in the excision rate between chemotherapy group and operation alone group (6/ 6:14/18).
