眼外伤合并迟发性硬膜外血肿的诊治分析

摘　　要：目的分析眼外伤合并初始无意识障碍硬膜外血肿的情况,以利及时诊治。方法对8例眼外伤患者合并迟发硬膜外血肿进行回顾性总结,就其发生机制、临床表现、诊治及发生的原因进行了分析。结果8例眼外伤包括眼球破裂,眶骨骨折,球后血肿,视神经损伤,初诊时无颅脑损伤的症状,头部CT平扫未见异常,伤后1～24小时患者出现颅高压症状,7例出现头痛、恶心、呕吐症状,1例出现意识障碍。复查头部CT出现迟发性硬膜外血肿。5例继续保守治疗,3例患者神经外科给予手术治疗。结论在眼外伤患者的治疗中应该考虑到迟发性硬膜外血肿的可能,眼科和神经外科医生应密切配合,抓住救治的最佳时机。Objective To evaluate the situation of the ocular injury with delayed extradural hematoma. Methods 8 cases of the ocular injury with delayed extradural hematoma were analyzed retrospectively. The mechanism of injury, clinical expression, diagnosis and treatment were studied. Results 8 cases with various ocular injuries included eyeball rupture, orbital fracture, postbulbarhematoma, and Optic nerve injury, which were no disturbance of consciousness and normal in the initial CT features, 7 cases complicating headache, nausea, vomit; 1 case complicating disturbance of consciousness. Delayed extradural hematoma were all apparent on the following up CT scanning, 5 cases conservative therapy; 3 caes operative therapy. Conclusions The delayed extradural hematoma shoud be considered in the treatment course of the ocular injury.
