中西医结合治疗幽门螺杆菌相关隆起糜烂性胃炎的临床观察

摘　　要：目的观察自拟平胃方联合抗幽门螺杆菌(Hp)三联疗法治疗Hp相关隆起糜烂性胃炎的临床疗效。方法将58例Hp相关隆起糜烂性胃炎患者随机分为2组。2组均抗Hp三联疗法(奥美拉唑20mg、克拉霉素0.5g、甲硝唑0.4g,均每日2次口服)治疗1周,1周后治疗组30例予平胃方,每日1剂,分2次饭后1h温服;对照组28例予铝碳酸镁咀嚼片1g,每日3次饭后1h口服。2组均3个月为1个疗程,停止治疗1个月后统计疗效,复查胃镜及检测Hp。结果2组治疗后各症状评分均降低,与本组治疗前比较差异均有统计学意义(P<0.01);且治疗组胃脘痛、饱胀、灼热感、泛酸、嗳气的改善情况优于对照组(P<0.05,P<0.01);治疗组总有效率90.0%,对照组67.9%,2组比较差异有统计学意义(P<0.01),治疗组临床疗效优于对照组;治疗组Hp根除率90.0%,对照组82.1%,2组比较差异无统计学意义(P>0.05)。结论自拟平胃方联合抗Hp三联疗法治疗Hp相关隆起糜烂性胃炎疗效确切。Objective To observe the clinical effect of treatment of erosive gastritis protuberance associated with Helicobacter pylori by self- made formula Pingwei combined with anti - Helicobacter pylori triple regimens. Methods 58 patients with erosive gastritis protuberance associated with Helicobacter pylori were randomly divided into treatment group （ n = 30） and control group （ n = 28）. After a week treatment of anti- Helicobacter pylori triple regimens （20mg Omeprazole, 0.5g Clarithromycin, 0.4g Metronidazole, twice a day） in two groups, patients in treatment group were treated by self- made formula Pingwei （a dose, twice a day）, and patients in control group were treated by Hydrotalcite （1g, thrice a day）. The therapeutic course was three montds in two groups. Results of gastroscope and Hp were compared with each other. Results Score of symptoms after treatment in two groups were lower than those before treatment （ P 〈 0.01 ）. Improvement of gastric abscess and belching in treatment group were superior to those in control group （ P 〈 0.05, P 〈 0.01） .The total effective rate is 90.0% in treatment group and 67.9% in control group, there is significant difference between them （ P 〈 0.01 ）. The eradication rate in treatment group （90.0%） was not higher than that in control group （82.1%, P 〉 0.05）. Conclusion There is obvious curative effect in treatment of erosive gastritis protuberance associated wim Helicobacter pylori by self- made formula Pingwei combined with anti - Helicobacter pylori triple regimens.
