P27在大肠癌、大肠腺瘤及正常大肠粘膜中的表达及临床意义

摘　　要：目的探讨P27Kip1在正常大肠粘膜、大肠腺瘤和大肠癌中的表达变化及与大肠癌发生、发展和转移的关系。方法采用免疫组化方法,分别检测20例正常大肠粘膜、22例大肠腺瘤和69例大肠癌中P27Kip1蛋白表达情况。结果P27Kip1蛋白在正常大肠粘膜、大肠腺瘤和大肠癌中阳性表达率分别为90.00%、86.00%和50.72%。大肠癌中P27Kip1蛋白表达阳性率显著低于大肠腺瘤和正常大肠粘膜(P<0.05)。P27蛋白的表达与肿瘤分化程度呈负相关(P<0.05),与临床其他病理因素均无相关性(P>0.05)。结论P27蛋白检测可作为评价大肠癌恶性程度的重要指标。Objective To investigate the relationship between the expression of P27 in the normal colorectal mucous membrane,eoloreetal adenoma, carcinoma and clinicopathologic features. Methods The expression of P27 was detected in 69 patients with colorectal carcinoma, 22 with colorectal adenoma and 20 normal colorectal mucous membrane by the immunohistochemical staining. Results The frequency of P27 staining in eoloreetal carcinoma was 50.72%, being significantly lower than that of in normal coloreetal mucous membrane and colorectal adenoma 90.00%, 86.00%, 50.72% respectively（P 〈 0.05 ）. A statistically significant difference in P27 staining was found in relation to the histologic grade（P 〈 0.05）. P27 expression was irrelated to several clinicopathologic features, including age, gender and gross type （P〉 0.05）. Conclusion The test of expression of P27 protein is helpful in assessing the malignant extent of coloreetal carcinoma.
