摘 要:目的探讨血管内皮生长因子(VEGF)及其受体VEGFR-2(KDR)和细菌L型感染在卵巢肿瘤中的表达及临床相关性研究。方法应用免疫组化、原位杂交和革兰染色等方法检测了120例卵巢肿瘤中的VEGF、KDR蛋白及mRNA的表达,以及细菌L型的检出率。并对97例卵巢乳头状癌和23例卵巢乳头状瘤主要临床资料和病理分级参数进行比较,用χ2检验进行统计学处理。结果VEGF、KDR蛋白及mRNA阳性表达率恶性肿瘤明显高于良性肿瘤(P<0.005)。细菌L型检出阳性率与卵巢良、恶性肿瘤差异无显著性(P>0.5)。VEGF、KDR蛋白及mRNA阳性表达以及细菌L型检出阳性率与卵巢乳头状癌的临床分期、病理分级和腹腔淋巴结有转移、腹水差异有显著性(P<0.001~0.05)。细菌L型阳性患者中VEGF、KDR阳性明显高于L型阴性患者中阳性表达,2组差异具有非常显著性(P<0.0001)。结论VEGF、KDR蛋白及mRNA在卵巢肿瘤中有不同程度的异常表达,与卵巢癌的临床分期、病理分级和浸润、转移呈正相关,L型感染极有可能成为诱发肿瘤因素之一,它们可能有协同致瘤作用。研究L型感染与卵巢肿瘤的关系,具有重要的临床应用价值。Objective To study the expressions of vascular endothelial growth factor and KDR acceptor and L-form bacteria infection in ovarian neoplasm and discuss their clinical correlation. Method Immunohistochemical staining, Hybridization in situ and Garm's stain were used to detect the expressions of VEGF,KDR protein and mRNA in 120 cases of ovarian carcinoma and to examine the positive rate of L-form bacteria in these tissues. The major clinical data and parameters of the pathological grades were compared in 97 cases of ovarian papillary carcinoma and 23 cases of ovarian papillary epithelioma. Result The positive rates of expressions of the VEGF, KDR protein and mRNA were obviously higher in malignant tumors than those in benign ones ( P 〈 0.005). The positive rate of examination of L-form bacteria had no significant difference between ovarian benign tumors and malignancy ( P 〉 0.5 ). The positive rates of expressions of the VEGF, KDR protein, mRNA and examination rate of L-form bacteria had significant difference between clinical stages, pathological grades, celiac lymph node metastasis and ascites in ovarian papillary carcinoma ( P 〈 0.001 - 0.05 ). The positive rates of the expressions of VEGF,KDR protein and mRNA were apparently higher in patients with L-form bacteria infection than those without infection. The difference between the two groups was highly significant (P 〈 0.0001 ). Conclusion VEGF, KDR protein and mRNA have certain abnormal expression in ovarian tumors. These expressions are in positive correlation with clinical stage, pathological grade, infiltration and metastasis. Infection of bacteria L-forms could be one of the inductive factors of the occurrence of tumors. The mutation and abnormal expression of VEGF, KDR, mRNA and Infection of bacteria L- forms possibly have a concurrent tumor-inducing function during the occurrence and development of tumors. It is clinically valuable to study the relationship between ovarian carcinoma and bacteria L-forms.
