幽门螺杆菌相关性消化性溃疡与中医辨证分型关系探讨

摘　　要：目的:通过分析146例幽门螺杆菌相关性消化性溃疡患者的临床资料,探讨溃疡病的证型特点。方法:对消化性溃疡患者进行分期、分型及舌象、脉象、胃镜观察。结果:脾胃湿热证、脾胃虚弱证多见于十二指肠溃疡,并多见于溃疡活动期,而肝胃不和证、胃阴不足证多见于消化性溃疡愈合期和瘢痕期。胃镜征象与中医各证型之间有密切联系,而溃疡各期与舌象、脉象之间也有密切联系。Hp阳性者以实证多见,在不同中医证型中分布例数依次为脾胃湿热证>肝胃不和证>脾胃虚弱证>胃阴不足证。结论:消化性溃疡其分期、分型与胃镜征象、HP感染及舌象、脉象关系密切。Objective ： To approach characteristic of pattern of syndrome by analyzing clinical data of 146 cases with peptic ulcer （PU） correlated to Helicobacter pylori （Hp）. Method ： Observe picture of the tongue, pulse tracings and gastroscope by staging peptic ulcer （PU）. Result： Syndrome of splenogastric hygropyrexia and syndrome of weakness of the spleen and stomach was always showed up in duodenal ulcer （ UD）, especially in active phase. Syndrome of incoordination between the liver and stomach and syndrome of asthenic stomach-yin was always showed up in healing stage and scarring stage. There were close association between gastroscope and each pattern of syndrome, and each stage of ulcer and picture of the tongue and pulse tracings. Positive Hp was showed up in sthenia syndrome, and Hp positive rate decreased progressively according to the order of syndrome of splenogastric hygropyrexia, syndrome of incoordination between the liver and stomach, syndrome of weakness of the spleen and stomach and syndrome of asthenic stomach-yin. Conclusion： There are close relationships among ulcer stage, grouping, gastroscope, Hp infection, tough picture and pulse tracings.
