摘 要:目的:研究B细胞活化因子(BAFF)和增殖诱导配体(APRIL)在多发性骨髓瘤(MM)患者骨髓单个核细胞(BMMCs)中mRNA表达特点及血浆中蛋白表达水平,并分析血浆中BAFF和APRIL水平与MM临床肿瘤负荷指标的相关性,初步探讨BAFF和APRIL对MM的发生、发展及预后的意义。方法:收集25例MM患者及10例正常对照的骨髓标本,获取BMMCs,并同步收取血浆。采用实时荧光定量PCR(RT-PCR)的方法检测BMMCs中BAFF和APRIL mRNA的表达,采用酶联免疫吸附试验(ELISA)法检测患者血浆中BAFF和A-PRIL的含量;使用相同方法,检测MM细胞株KM3中BAFF及APRIL的基因及蛋白表达特点。结果:①BAFF和APRIL基因和蛋白在MM组中表达升高,与对照组比较差异有统计学意义(P<0.01);②BAFF和APRIL基因和蛋白在MM初治组及未缓解组中表达升高,与缓解组比较差异有统计学意义(P<0.05),初治组与未缓解组比较差异无统计学意义;③BAFF和APRIL蛋白水平与患者外周血中ALB呈负相关(P<0.05),与外周血中β2微球蛋白(β2-MG)、免疫球蛋白(Ig)浓度及骨髓中浆细胞百分比均呈正相关(均P<0.05),MM血浆中BAFF和APRIL浓度之间呈正相关(P<0.01)。结论:MM中BAFF和APRIL的基因及蛋白表达升高,MM组内初治组、未缓解组均高于缓解组,血浆中BAFF和APRIL含量与MM肿瘤负荷呈正相关。BAFF及APRIL的表达特点可能与MM的发生、发展有关,可作为评判MM预后的新指标。Objective:To determine the mRNA and protein expressions of B-cell activating factor (BAFF) and a proliferation-inducing ligand (APRIL) in multiple myeloma (MM) and analysis the relationship between BAFF (or APRIL) and clinical tumor burden indicators, and explore thcir clinical significance. Method: Bone marrow mononuclear cells were isolated from 25 patients with MM and from 10 normal people as control. Real-time quan- titative PCR (RT-PCR) was performed to evaluate the mRNA expression of BAFF and APRIL. ELISA was used to detect BAFF and APRIL protein expressions in serum. Result:(DBoth mRNA and protein expressions of BAFF and APRIL were higher in MM group than that in control group (P〈0.01). (1)In MM group, BAFF and APRIL expressions in newly diagnosed group and non-remission group were higher than remission group (P〈0.05) ;and there was no difference between newly diagnosed group and non-remission group (P〈0.05).(2)The protein expression of BAFF was positively associated with APRIL expression (P〈0.05). Both APRIL and BAFF protein levels were associated with a higher serum albumin (P〈0.05), β2-microglobulin (β2-MG) (P〈0.05), immuno- globulin (P〈0. 05) and marrow percentage of plasma cell (P〈0.01). Conclusion: BAFF and APRIL were highly expressed in MM; and in newly diagnosed group and non-remission group, the expressions of BAFF and APRIL were higher. The expression levels of BAFF and APRIL were positively correlated with the tumor load, which in- dicated that both of them could be a new prognosis indicator of MM.
