幽门螺杆菌感染与胃癌的关系研究

摘　　要：为进一步阐明Hp感染与胃癌的关系，采用群组病例一对照方法，对胃癌组70例及正常对照组34例进行快速尿素酶试验、组织学Giemsa染色和Hp培养等检查。结果：①Hp在胃癌组检出率明显高于对照组，比数比（Oddsratio,OR）＝11．94，95％CI为4．32～33．01，P＜0．01。②Hp感染与胃窦、胃体部、组织分型为肠型之胃腺癌密切相关，P＜0.01，OR值分别为38．75，27.50及26．25；95％CI各为11．88～126．38，6．60～114．00和8．80～81．70。Hp感染与贲门部、弥漫型胃腺癌关系不大，P＞0．50、P＞0．10；OR分别为1．41、2．92；95％CI各为0．28～7.14，0．77～11．02.提示Hp感染是胃癌发生的重要病因。Recently helicobacterium pylori (Hp) infection has been incriminated in the pathogenesis of gastric cancer. In order to study the correlation between Hp infection and gastric cancer, we used rapid urease test, histology (Giemsa) and culture Of Hp to detect the Hp infection of gastric biopsy specimens jn 70 patients with gastric cancer and 34 normal persons for control. The results showed that:①The incidence of Hp infection in gastric cancer group is significantly higher than tha of control group (OR=11. 94, 95% CI=4. 32 ~33. 01. P< 0.01 ). ②There is strong evidence that Hp infection is associated with gastric edenocarcinoma located at gastric antrum or body, and their histomorphology are of intestinal type (P<0.01), OR 38. 75, 27. 50, 26. 25; 95% CI 11. 88~126. 38, 6. 60 ~114. 00, 8. 80 ~ 81. 70, there is no correlation between Hp infection and the gastric adenocarcinoma situated on gastric cardia, and its histomorphology is of diffude type>0. 50, P>0. 10, OR 1. 41, 2. 92, 95% CI 0. 28~7. 14, 0. 77~11. 02,the study suggested that the Hp infection may play an important role in the pathogenesis of gastric cancer.
