沟槽辅助固位后牙粘结桥的临床初步观察

摘　　要：目的本实验旨在对一颗后牙缺失的Kinnedy Class Ⅲ患者用沟槽辅助固位后牙粘结桥修复，并作短期随访观察，评价其临床效果。方法选取2004年7月至2008年1月来我科就诊的51例患者，用沟槽辅助固位的后牙粘结桥修复。所有60颗患牙：为非游离端的一颗后牙缺失，缺牙间隙正常，邻牙不松动，无牙龈炎、根尖周炎，釉质发育良好，袷面釉质磨耗少，咬袷关系正常。制作沟槽辅助固位固定桥，用Calibera树脂粘结剂进行粘结。然后患者每隔3个月复诊，评价其临床效果：固位体松动，修复体脱落、移位，基牙敏感，基牙牙龈出血指数。结果51例患者中，2例发生固位体松动、修复体移位、脱落，5例基牙敏感，修复前后PI差异无统计学意义。结论在平均16个月的观察期内，用沟槽辅助固位后牙粘结桥修复的固定桥临床效果良好，经过严格的病例选择，沟槽辅助固位后牙粘结桥可望成为永久修复体，但仍需要长期的临床随访观察。Objective To evaluate the acceptability of resin - bonded fixed partial denture with a modified design used in Kinnedy Class Ⅲ patients over a short period. Methods Sixty missing teeth were restored with resin - bonded fixed partial denture with slots design. The dentures were luted with Calibera Resin Cement according to the manufacture＇ s recommendation. The parameters considered as clinical failure retainers were displacement, detachment. The patients were reevaluated ever 3 months. Results A total of 60 postior resin - bonded fixed patial dentures were made for 51 patientso The mean follow- up period was 16 months. Two prosthesis decementation were observed and the debonding rate was 3.3 %. Five patients had abutment dentin sensitivity after restoration （ 8.3 % ）. There were no statistical significance of PI before and after rehabilitation. Conclusions Over a period 16 months, the resin - bonded fixed partial dentures showed good clinical results. Permenent results of clinical service indicated that this modified design dentures maybe become a long -lasting restorations after a carefully case selection. Of course it needs further clinical follow - up.
