中期妊娠唐氏综合征血清筛查的临床价值

摘　　要：目的探讨中期妊娠唐氏综合征血清筛查的临床价值。方法采用贝克曼库尔特全自动化学发光酶免分析仪及原装试剂,对中期孕妇血清中的孕妇血清、羊水及胎血进行甲胎蛋白(AFP)、人绒毛膜促性激素β亚基(β-HCG)及游离雌三醇(uE3)检测,并将孕妇的相关信息和检测的AFP、β-HCG、uE3结果输入筛查软件中,进行三联检测的结果运算,得出一个概率指标。结果①唐氏综合征筛查结果:20岁以下年龄组阳性率为零,20～25岁年龄组阳性有5例,阳性率为0.72%,25～30岁年龄组阳性有17例,阳性率为1.48%,30岁以上年龄组的阳性有22例,阳性率为4.35%。②开放性神经管缺陷筛查结果,20岁以下年龄组阳性有1例,阳性率为3.70%,20～25岁年龄组阳性有7例,阳性率为1.01%,25～30岁年龄组阳性有14例,阳性率为1.22%,30岁以上年龄组阳性有6例,阳性率为1.19%。③18三体综合征(Trisomy18)筛查,唯有25～30岁年龄组孕妇有2例阳性,其阳性率为0.17%。其他年龄组均无阳性。结论在中期孕妇进行唐氏综合征三联筛查是必要的,筛查阳性的孕妇一定要作确诊试验。对于确诊阳性孕妇,应做耐心的解释工作,继而终止妊娠,这对提高人种素质,促进国民经济发展和社会的稳定十分必要。Objective Exploring of the clinical significance of down＇s syndrome screening in the middle period of pregnancy. Methods Beckman - Coulter auto chemical illuminating immune analyser and its reagents are used to measure the serum level of AFP, β - HCG and uE3 of women with the middle period of pregnancy. The related information of the pregnant women and the combined results of AFP, β - HCG and uE3 are put into and are processed by screening software, therefore a Probability Index is produced. Results ①Down＇s syndrome screening results ： positive rate is zero for the group younger than 20 years old ;positive rate is 0.72% （5 cases are positive）for the group with the age between 20 -25 years old ; Positive rate is 1.48% （ 17 cases are positive） for the group with the age between 25 -30 years old ; Positive rate is 4.35 % （22 cases are positive） for the group older than 30 years old ;②Open neural tube defect screening results ： positive rate is 3.70% （ 1 case positive） for the group younger than 20 years old;Positive rate is 1.01% （7 cases are positive ）for the group with the age between 20 -25 years old;Positive rate is 1. 22% （ 14 cases are positive）for the group with the age between 25 ~ 30 years old;Positive rate is 1.19% （6 cases are positive）for the group older than 30 years old ;③Screening results of Trisomy 18 ： only 2 cases are positive within the group with the age between 25 - 30 years old, positive rate is 1.19%, all others are negative. Conclusion It is significant to do the combined screening of down＇s syndrome in the middle period of pregnancy, Pregant women with the positive result of screening must be experiment for accurate diagnosis. For the pregant women with the positive result and accurate diagnosis, A persuasive and patient explanation is necessary to stop the pregnancy. This is very critical to improve the Chinese population quality and promote the development of national economy and social stability.
