摘 要:背景:表没食子儿茶素没食子酸酯(EGCG)和叶酸对消化道肿瘤均具有化学预防作用。目的:研究EGCG和叶酸对N-甲基-N'-硝基-N-亚硝基胍(MNNG)诱导的大鼠消化道肿瘤的化学预防作用及其作用机制。方法:159只健康雄性Wistar大鼠分为肿瘤模型组(M组,MNNG100mg/L自由饮用)、EGCG对照组(E组,EGCG12.5mg/d)、叶酸对照组(F组,叶酸1mg/d)、EGCG干预组(ME组,MNNG+EGCG)、叶酸干预组(MF组,MNNG+叶酸)、EGCG+叶酸干预组(MEF组,MNNG+EGCG+叶酸)和正常对照组(C组)。第29周停止MNNG和药物干预,第44周终止实验,处死各组大鼠,比较各组消化道肿瘤发生情况,行肿瘤组织Ki-67和原位凋亡检测。结果:与M组相比,MEF组肿瘤发生率显著降低(P=0.011);ME组和MEF组肿瘤组织Ki-67阳性率显著降低(P=0.038和P=0.009);ME组、MF组和MEF组肿瘤组织细胞凋亡率显著增高(P=0.000、P=0.003和P=0.000)。各干预组间上述指标差异均无统计学意义。结论:EGCG与叶酸联合应用对MNNG诱导的大鼠消化道肿瘤具有化学预防作用,但并不比两者单用更为有效;两者联用对肿瘤细胞增殖和凋亡的影响亦与单用无明显差异。Epigallocatechin-3-gallate (EGCG) and folic acid have been used for the chemoprevention of gastrointestinal tumors. Aims: To investigate the chemopreventive effect and mechanisms of EGCG and folic acid on N- methyl-N'-nitro-N-nitrosoguanidine (MNNG)-induced gastrointestinal cancer in rats. Methods: One hundred and fifty- nine healthy male Wistar rats were divided into seven groups: group M, MNNG 100 mg/L drinking freely; group E, EGCG 12.5 mg/d; group F, folie acid 1 mg/d; group ME, MNNG plus EGCG; group MF, MNNG plus folio acid; group MEF, MNNG plus EGCG and folic acid; group C, normal control group. At 29th week, MNNG and EGCG and/or folic acid intervention were ceased; at 44th week, all rats were sacrificed and examined. Occurrence of cancer was evaluated by histology; Ki-67 in cancerous tissues and in situ apoptosis were determined. Results: Compared with group M, the cancer incidence of group MEF decreased significantly (P=0.011); Ki-67 expression in cancerous tissues of group ME and MEF also decreased significantly (P=0.038, P=0.009), while apoptosis of group ME, MF and MEF increased significantly (P= 0.000, P=0.003, P=0.000). No statistical differences were found among these three intervention groups. Conclusions: EGCG combined with folic acid have obvious chemopreventive effect on gastrointestinal cancer induced by MNNG in rats, but the effect is not greater than that of EGCG or folic acid alone. Also, the effect of combined EGCG and folic acid on proliferation and apoptosis of cancer cells is similar to that of EGCG or folic acid alone.
