咪达唑仑清醒镇静无痛内镜检查的临床应用

摘　　要：背景：目前国内多采用单独或联合使用丙泊酚或芬太尼等药物产生深度镇静或基础麻醉的方法进行无痛内镜检查,实施时必须有麻醉科医师配合,且费用昂贵。目的：总结单独应用咪达唑仑行清醒镇静无痛内镜检查的合理用量、效果、安全性和临床应用价值。方法：纳入佛山市第二人民医院1997年8月～2008年2月单独应用咪达唑仑行清醒镇静无痛内镜检查的患者,对其一般情况、药物用量、成功率等行回顾性分析。结果：共28600例患者单独应用咪达唑仑行清醒镇静无痛内镜检查,其中清醒镇静胃镜检查22100例,平均用量为3.0mg;清醒镇静结肠镜检查6500例,平均用量为4.0mg。清醒镇静内镜检查的成功率为100%,98%的患者对检查过程完全遗忘,很满意和满意的比例为95%,行走恢复时间平均为（13±5）min,无一例患者发生严重并发症。结论：单独应用咪达唑仑行清醒镇静无痛内镜检查具有无痛效果显著、恢复迅速、安全、费用低廉的优点,适用于广大基层医院。Background： Propofol or fentanyl alone or in combination is commonly used in painless endoscopy for deep sedation or basal narcosis in domestic, but it needs the cooperation of anesthesiologist and is expensive. Aims： To summarize the dosage, efficacy, safety and value of clinical application of midazolam alone for conscious sedation in painless endoscopy. Methods： Patients who underwent conscious sedation with midazolam alone for painless endoscopy from Aug. 1997 to Feb. 2008 in the Second People＇s Hospital of Foshan were recruited. The general status of patients, dosage of midazolam and success rate, etc. were analyzed retrospectively. Results： A total of 28 600 patients were examined by painless endoscopy under conscious sedation with midazolam alone, including 22 100 patients for gastroscopy and 6500 patients for colonoseopy, and the average dosage of midazolam was 3.0 mg and 4.0 mg, respectively. The success rate of conscious sedation for endoscopy was 100%. The percentage of patients with total amnesia was 98%. Ninty-five percent of patients felt very satisfactory or satisfactory. The average recovery time for walking was （13±5） minutes. No patients had severe complications. Conclusions： Conscious sedation with midazolam alone for painless endoscopy has the merits of painlessness, quick recovery, safety and less expensive, and is suitable for wide use in primary hospitals.
