肝硬化病人左右半肝形态及其相关因子表达变化

摘　　要：目的探讨乙型肝炎肝硬化病人左右半肝形态及肝组织标本肝硬化相关因子基质金属蛋白酶-2(MMP2)、碱性成纤维细胞生长因子-2(bFGF2)的表达。方法对30例乙型肝炎肝硬化致门脉高压征行脾切除术和乙型肝炎肝硬化失代偿行肝移植手术病人行CT检查,手术中标本摄片,留取形态学资料。同时留取病人肝组织标本,进行苏木精-伊红染色,根据0～4期标准判断肝纤维化程度,并行免疫组化染色检测MMP2和bFGF2的表达。结果肝硬化病人右半肝萎缩较左半肝萎缩明显;苏木精-伊红染色显示右半肝肝硬化较左半肝明显。病例组左、右半肝MMP2的表达明显高于对照组左、右半肝的表达(χ2=19.45、17.60,P<0.01);病例组右半肝MMP2的表达明显高于左半肝(χ2=17.37,P<0.01)。病例组左半肝bFGF2的表达与对照组比较无统计学差异,而右半肝bFGF2的表达明显高于对照组(χ2=5.80,P<0.05);病例组右半肝bFGF2的表达明显高于左半肝(χ2=11.09,P<0.01)。结论肝硬化时左右半肝纤维化程度存在明显差异,MMP2和bFGF2的表达可能对病变程度起作用。Objective To explore the morphology of left and right hemihepar and the expressions of cirrhosis-related factors-matrix metalloproteinase 2 （MMP-2） and basic fibroblast growth factor 2 （bFGF-2）-in patients with type-B-hepatitis cirrhosis. Methods CT scan and intraoperative specimen photographs were done in 30 patients undergoing splenectomy for portal hypertension or liver transplantation for liver in decompensation stage caused by type-B-hepatitis cirrhosis. The liver tissue was taken for HE staining and estimating the extent of hepatic fibrosis based on 0- Ⅳ-stage standard, the expressions of. MMP2 and bFGF2 were detected by immunohistoehemistry method. Results Hepatatrophy was observed more seriously in the right hemihepar than the left; HE stain showed cirrhosis was also marker in the right hemiliver. The expression of MMP2 of left and right hemiliver in the patient group was higher than that in the control （χ^2 = 19.45,17.60;P〈0.01）. In patient group, the MMP2 was higher in the right hemiliver than the left （χ^2 = 17.37 ,P〈0.01）. The difference of bFGF2 expression in left hemiliver between the patient group and the eontrol was not significant, while the expression in right hemiliver was higher than that in the control （χ^2 = 5.80, P〈0.05）. In patient group, the expression of bFGF2 in right hemiliver was higher than the left （χ^2 =11.09,P〈0.01）. Conclusion A marked difference of liver fibrosis exists between left and right hemiliver, the expressions of MMP2 and bFGF2 may play a role of this difference.
