41例脊柱浆细胞瘤中E-cadherin的表达与临床病理特征的关系

摘　　要：目的探讨脊柱浆细胞瘤中E-cadherin蛋白的表达，及其与脊柱浆细胞骨髓瘤生物学行为的关系。方法应用免疫组织化学S-P法检测41例脊柱浆细胞瘤及14例脊柱良性单纯性骨囊肿中E-cadherin蛋白的表达。结果41例脊柱浆细胞瘤中E-cadherin蛋白表达阳性率为46.34％，而良性骨囊肿中E-cadherin蛋白表达呈全阴性，存在统计学差异（P％0.01），脊柱浆细胞骨髓瘤中E-cadherin表达阳性组总生存率明显高于阴性组（P〈0.01）。本研究还发现脊柱浆细胞骨髓瘤中E-cadherin表达与轻链分型有关，未分泌型（6／9，66.7％）和K型（4／12，66.7％）阳性率均较X型（5／20，25％）高，且存在统计学差异（P〈0.05）。但E-cadherin的表达与脊柱浆细胞瘤的临床分型、免疫球蛋白分型、性别及年龄等，均无关（P〉0.05）。结论提示联合检测E-cadherin和轻链分型有助于对浆细胞骨髓瘤侵袭能力的评估，对浆细胞骨髓瘤的预后判断可能具有一定的临床意义。Objective To investigate the expression level of E-cadherin in spinal plasmacytoma and the relationship between the E eadherin expression and the clinicopathological parameters of spinal plasmaeytoma. Methods The expression of E-cadherin was detected in 41 spinal PCMs and 14 simple bone cysts by using S-P（streptavidin-perosidase ） immunohistochemical staining. Results In spinal plasmacytoma, the positive expression rate of E-eadherin （46.34%） was higher than that in simple bone cyst tissues （all negative） （P〈0.01）. And the expression of E-cadherin had no obvious relationship with patients age, gender, clinical or immunoglobulin classification （P〉0.05）, but had relationship with the classification of light chain, the expression rate in lambda chain was lower than that in the others （P〈0.05）. We also found the E-cadherin negative patients had a higher overall survival rate than those had strong E cadherin expression （P〈0.05）. Conelsuion E-cadherin expressed higher in spinal PCM than in simple bone cyst tissues and its expression had relationship with the light chain classification in spinal plasmacytoma. The patients with no stain of E cadherin maybe have a poor prognosis.
