摘  要:目的探讨扭动模式白内障超声乳化术的临床应用效率以及对角膜内皮细胞密度的影响,评价其安全性。方法对行白内障超声乳化联合人工晶状体植入术的年龄相关性白内障Ⅲ-Ⅳ级核患者99例125只眼进行前瞻性研究,手术随机分为扭动模式(Torsionalmode)超声乳化与爆破模式(UltrasonicBurstMode)常规超声乳化两组,记录术中所用的超声乳化时间(UltrasoundTime,UST)、累积释放能量(CumulativeDissipatedEnergy,CDE)、术前与术后3个月角膜中央区内皮细胞密度的变化,进行不均衡析因设计的方差分析。结果(1)扭动模式组所使用的UST低于爆破模式组。(2)扭动模式组所使用的CDE低于爆破模式组。(3)术后3个月扭动模式组角膜内皮细胞的丢失低于爆破模式组。结论扭动模式超声乳化与爆破模式传统超声乳化相比,术中使用的超声时间短、累积超声能量低,可减少对角膜内皮细胞的损伤,提高了白内障超声乳化手术的效率及安全性。Objective To study the efficiency and effect on corneal endothelium density of torsional mode for cataract phacoemulsification.Methods 125 eyes of 99 age-related cataract patients were randomly assigned to underwent phacoemulsification either with conventional burst mode or torsional mode in Shandong Provincial Hospital.All patients underwent nuclear sclerosis grading and central endothelial cell count(CECC) before operation.Ultrasound Time (UST)and Cumulative Dissipated Energy (CDE)were recorded.Observed CECC before and at 90 days after surgery.Results For both nuclear sclerosis Grade III and IV cataract,a)the mean UST was significantly lower for the Torsional Group (P 〈0.01 ) ;b)the mean CDE was significantly lower for the Torsional Group (P 〈0.01 ) ;c)the CECC was significantly higher in the Torsional group (P 〈0.01 )90 days after surgery.Cunelusion Comparing with conventional ultrasound burst mode, torsional ultrasound mode uses shorter UST,lower CDE and causes less damage to the corneal endothelial cells.
