摘  要:目的探讨超生乳化吸出及后房型人工晶状体植入小梁咬切术(三联手术),联合丝裂霉素治疗青光眼合并白内障的临床效果。方法应用超声乳化吸出、后房型人工晶状体植入联合小梁咬切术对58例(58只眼)青光眼合并白内障的患者进行手术治疗,术后随访12~18月,分别观察术后视力、眼压、滤过泡形成和并发症发生的情况。结果术后除8只眼经按摩联合结膜下注射5-FU治疗后眼压恢复正常外,其余眼压均在正常范围内。术后视力均有不同程度的提高。滤过泡均扁平弥散,属功能性滤过泡。术后3只眼角膜内皮皱褶,5只眼虹膜少许后粘连,并有人工晶状体前膜形成,1只眼脉络膜脱离经保守治疗后病情恢复正常,余无严重并发症。结论超声乳化吸出、后房型人工晶状体植入联合小梁咬切术是治疗青光眼合并白内障的安全、有效的方法。Objective To discuss the clinical effects of Phacoemufication & posterior chamber intraocular lens combined with trabeculectomy inside the tunnel.Methods Phacoemulsification and intraocular lens implantation combined with trabeculectomy inside the tunnel was performed on 58cases (58eyes)of glau- coma combined cataract.The patients were followed for 12 to 18 months.Post-operative visual acuity, intraocu- lar pressure and complications were analyzed.Results After operation, 8eyes cured with massage combined with subconjunctiva injection with 5-FU.Other patients gained good control of ocular pressure.The visual acuity was improved in all cases postoperatively.AU filter bleb were functional bleb and its appearance was fiat and dispersed.After operation, there was corneal endothelium reductus in 8eyes, a little posterior synechia of the iris accompanied with formation of frontal membrance of artificial lens in 5 eyes, choroidal detachment in 1 eye.All this had been cured by conservative treatment.There was no other serious complication.Conclusions The pha- coemulsification and intraocular lens implantation combined with trabeculectomy inside the tunnel is safe and efficient method in treating glaucoma combined cataract.
