原发性肝癌多支供血在治疗中的重要性

摘　　要：目的：探讨肝癌的多支血供对ＴＡＣＥ治疗效果的作用。方法：对１３４例原发性肝癌采用Ｓｅｌｄｉｎｇｅｒ技术进行造影及ＴＡＣＥ治疗。结果：治疗中发现５０例（３８．８％）存在多支供血，其中副肝动脉供血１２例（８．９６％），侧支开放供血３８例（２８．３６％）。对５０例多支供血的肝癌只作单纯肝动脉ＴＡＣＥ治疗与多支动脉ＴＡＣＥ治疗结果比较，肿瘤缩小率有显著差异（Ｐ＜０．０１）。结论：通过对１３４例患者动脉造影发现：肝癌的侧支供血与肿瘤的部位密切相关，肝癌的多支供血动脉逐一进行ＴＡＣＥ治疗其效果明显优于单纯肝动脉ＴＡＣＥ治疗（Ｐ＜０．０１）。Objective:To investigate the effect of multiple feeding vessels of hepatocellular carcinoma on TACE.Methods:134 cases with hepatocellular carcinoma were treated by arteriography and TACE using seldinger techniqae.Results:Multiple feeding vessels were found in 50 cases(38.9%),of them 12 cases had hepatic arteriae collateralis(8.96%),38 cases were of open-feeding vessels (28.36%),50 cases with hepatocellula carcinoma who were of multiple feeding vessels were treated by the simple hepatic arteries,TACE and multiple feeding arteries TACE one after another,and the rusult was compared.The decreased rate in size of tumor was significant difference(P<0.01).Conclusion:arteriography in 134 cases we found that there is a closerelation between the lateral feeding vessel in hepatic carcinoma and the position of carcinoma.The effect of TACE through multple feeding vessels one by one is better than only by simple hepatic arteriae(P<0.01)
