结直肠癌患者术前SAA、CRP、CEA和CA19-9水平的临床诊断价值

摘　　要：目的探讨结直肠癌患者血清淀粉样蛋白A(SAA)与C反应蛋白(CRP),癌胚抗原(CEA),CA19-9的相关性和SAA对于结直肠肿瘤术前分期的价值。方法纳入结直肠癌患者41例,测定术前SAA、CRP、CEA和CA19-9水平,并与术后病理分期结果进行对比分析。结果SAA和CRP有高度相关性,相关系数r=0.90。以诊断有转移的结直肠癌(进展期结直肠癌)为目标,建立SAA的ROC曲线。ROC曲线下面积AZ=0.831,以SAA=4.045mg/L为临界点,得到SAA敏感度为80.0%,特异度为76.2%。结论结直肠肿瘤患者的SAA和CRP水平高度相关,说明肿瘤转移与炎症反应有关联。SAA对转移性结直肠癌有较好的诊断价值,可用于术前分期和预后评估。Objective To investigate the relationship among serum amyloid A protein （SAA）, C-reactive protein（CRP）, carcinoembryonic antigen（CEA） and carbohydate antigen 19-9（CA19-9） and to assess the value of applicaton of SAA in the preoperative staging of colorectal cancer patients （CRC ）. Methods Levels of SAA, CRP, CEA and CA19-9 in CRC patients were measured preoperatively, and their relationship with tumor staging was analyzed. Results SAA shows high correlation with CRP, of which the correlation coefficient is 0.90.When SAA = 4.045mg/L was set as the cut off point,the ROC curve was built to diagnose the advanced colorectal cancer, of which the area under curve was 0.831 and sensitivity and specificity were 80% and 76.2% respectively. Conclusion The high correlation of SAA and CRP in CRC patients demonstrates that the metastasis of tumor associate with the inflammation.The diagnostic value of SAA on metastatic colorectal cancer is a potentially powerful biomarker for preoperative staging and prognosis.
