精神分裂症不同症状的MRI对照研究

摘　　要：选择４０例住院精神分裂症病人（其中阳性症状组３０例，阴性症状组１０例），和正常对照组２０例检测头颅ＭＲＩ。结果：精神分裂症脑结构异常率为３２．５％，其中脑软化７．５％，海马回梗塞１７．５％，脑白质变性１５％，额颞皮层萎缩２５％。阴性症状组病人的两侧脑室前角，三角区间距，三脑室宽径，大脑纵裂前半间距，左、右侧裂池宽距比正常对照组明显扩大。阳性症状组病人的两侧脑室三角区间距，三脑室宽径亦比正常对照组明显扩大。阴性症状组与阳性症状组比较，阴性症状组病人的大脑纵裂前半间距，左、右侧裂池宽距，两侧脑室前角间距明显扩大。且两侧脑室前角扩大，大脑纵裂前半间距，左、右纵裂池宽距扩大。Objective:To compare the MRI findings between schizophrenic patients with positive or negative symptoms and normal controls. Method:30 schizophrenic patients with positive symptoms,10 with negative symptoms and 20 normal subjects were studied with MRI of the brain. Results:An overall 325% of structural abnormality was found in the schizophrenic subjects compared with the control group.Abnormal findings included encephamalacia 75%,hippocampal formation infarction 175%,white matter degeneration 15% and frontal/temporal lobe atrophy 25%.The clearance between the anterior homs triangular areas of lateral ventricles width of anterior half of longitudinal fissure lateral fissures and the third ventricles were significantly larger in schizophrenic patients with negative symptoms than controls The clearance between triangular areas of lateral ventricles and width of third ventricles were significantly larger in schizophrenic patients with positive symptoms than controls.The clearance between anterior horns of lateral ventricles,width of anterior half of longitudinal fissure,and lateral fissures were significantly larger in those with negative than positive symptoms. Discussion:There was significant correlation between duration of illness and cerebral abnormalities that include enlargement of clearance between the anterior horns width of anterior half of longitudinal fissure and width of lateral fissures.
