摘  要:目的:探讨CD44v5、CD44v6、CD44s及LN-R的表达异常和葡萄胎恶变的关系及在预测葡萄胎恶变方面的价值。方法:采用针对4种产物的单克隆抗体,S-P法免疫组织化学染色,回顾性分析了经随访证实发生恶变的葡萄胎49例(恶变组)和未发生恶变的葡萄胎72例(非恶变组)中4种产物的表达情况。结果:恶变组CD44v5的表达程度显著低于非恶变组(P<0.01);恶变组CD44v6表达程度显著高于非恶变组(P<0.01)。进一步以4种产物的表达程度为变量,进行Logistic回归判别分析,表明4种产物联合应用对葡萄胎恶变的预测敏感性率为60.5%,特异性率为84.3%,而Logistic逐步回归判别分析显示,CD44v5和CD44v6联合应用时,对葡萄胎恶变的预测敏感性率为60.5%,特异性率为88.2%。结论:CD44v5和CD44v6的表达可作为预测葡萄胎恶变的新指标。Objective: To explore the relationship between abnormal expression of CD44s, CD44v5, CD44v6, laminin receptor andthe malignant transformation of hydatidiform mole. Methods : The levels of CD44 s, CD44 vS, CD44 v6 and laminin receptor in 49 cases of hydatidiform mole with malignant transformation (malignant transformation group) and 72 cases of hydatidiform mole without malignant transformation (non -malignant transformation group) were detected by SP immunohistochemistry staining. Results: The level of CD44 v5 in malignant transformation group was significantly lower than that in non -malignant transformalion group (P 〈 0. 01 ) ; while the level of CD44 v6 in malignant transformation group was significantly higher than that in non - malignant transformation group (P 〈 0. 01 ) . Logistic regression analysis indicated that the sensitivity of combined use of the 4 products was 60. 5% , the specificity was 84. 3% ; the sensitivity of combined use of CD44v5 and CD44v6 was 60. 5% , the specificity was 88.2%. Conclusion: The expression of CD44v5 and CD44v6 can be used as a new index in the prediction of malignant transformation of hydatidiform mole.
