食管鳞癌中CD44v6和E-cadherin的表达

摘　　要：目的探讨CD44v6和E-cadherin在食管鳞癌中的表达及其意义。方法应用免疫组织化学SP法检测15例正常食管鳞状上皮组织、20例癌旁上皮内瘤变组织、60例食管鳞癌组织中CD44v6和E-cadherin的表达。结果在正常食管鳞状上皮组织、癌旁上皮内瘤变组织和食管癌组织中,CD44v6阳性表达率分别为13.3%(2/15)、45.0%(9/20)和83.3%(50/60),三者间差异有显著性(P<0.01);E-cadherin的阳性表达率分别为86.7%(13/15)、55.0%(11/20)和23.3%(14/60),三者间差异有显著性(P<0.01)。CD44v6的高表达及E-cadherin的低表达与食管癌的浸润深度、淋巴结转移、临床病理分期、三年生存率均呈正相关(P<0.05),E-cadherin表达与肿瘤的分化程度有明显相关性(P<0.05),而CD44v6表达与肿瘤的分化程度无明显相关性(P>0.05)。食管癌中CD44v6表达与E-cadherin表达呈负相关(x2=9.02,P<0.05,r=-0.362)。结论CD44v6和E-cadherin的表达可能具有负调节的协同作用。CD44v6和E-cadherin的表达与食管癌发生发展、浸润转移和预后密切相关,可作为预测食管癌侵袭转移和估计预后的重要参考指标。Objective To investigate the clinical significance of CD44v6 and E-cadherin in esophageal squamous cell carcinoma. Methods Expression of CD44v6 and E-cadherin in 15 cases of normal esophageal mucosa, 20 cases of peritumorous intraepithelial neoplasia tissues and 60 cases of esophageal squamous cell carcinoma were determined by SP immunohistoehemical technique. Results In normal esophageal mucosa, peritumorous intraepithelial neoplasia tissues and esophageal squamous cell carcinoma, the positive rate of CD44v6 was 13.3% （2/15） ,45.0% （9/20） and 83.3% （50/60） respectively, there was significant difference between the three groups（P 〈0. 01 ）; the positive rate of E-cadherin was 86.7% （13/15）, 55.0% （11/20） and 23.3% （14/60）, there was significant difference between the three groups（ P 〈0. 01 ）. The high level expression of CD44v6 and the low level expression of E-cadherin were positively correlated with depth of infiltration, lymph node metastasis, clinical pathological staging and survival rate of three years in esophageal carcinoma（P 〈0. 05）. The expression of E-cadherin was related with differentiate degree of tumors（ P 〈 0. 05 ）, but the expression of CD44v6 not（P 〉 0. 05 ）. And it was positively associated with the expression of CD44v6 and negatively with that of E-cadherin in esophageal carcinoma （χ^2 = 9. 02, P 〈0. 05, r = -0. 362）. Conclusion These data show a correlation between the CD44v6 and E-cadherin expression . The expression of CD44v6 and E-cadherin is correlated with the development, invasion, metastasis and prognosis in eaophageal squamous cell carcinoma. CD44v6 and E-cadherin can be regarded as important signs of invasion, metastasis and prognosis.
