Ⅰ、Ⅱ期乳腺癌组织中p27^(Kip1)的表达及其临床意义

摘　　要：目的:乳腺癌患者的预后以肿瘤大小、淋巴结转移、激素水平等指标来评价,但结果并不理想,为了更加合理的指导和评价乳腺癌预后,提高患者生存期,探讨p27Kip1在临床Ⅰ、Ⅱ期乳腺癌组织中的表达对预后的影响及其与临床病理特征之间的关系。方法:应用EnVision荧光免疫组化染色检测106例Ⅰ、Ⅱ期乳腺癌患者细胞中p27Kip1的表达。结果:106例乳腺癌组织中p27Kip1核内低表达率为57.55%。p27Kip1核内高表达者与低表达者生存率差异显著有统计学意义(P<0.05)。28例出现了p27Kip1细胞质内表达,表达率为26.50%,细胞质内表达较核内表达预后差。p27Kip1在早期乳腺癌细胞的表达与年龄、月经状况、病理类型、肿瘤大小、临床分期、激素受体情况等无关(P>0.05),与淋巴结转移情况和组织分化程度有关(P<0.05)。结论:早期乳腺癌细胞中出现了p27Kip1的表达下降和错位分布,核内高表达者预后较好,p27Kip1可能是早期乳腺癌预后指标之一。p27Kip1表达与淋巴结转移和组织分化程度有关。Objective ：Tumor size, lymphnode metastasis, hormone level and so on have been the prognosis predictors of breast cancer, but they are not ideal. This study is to investigate the influence of the expression of p27^Kip1 in early breast cancer prognosis and its relationship with clinical feature. Methods ：Archival early breast carcinomas （n = 106 ）, were analyzed for p27^Kip1 expression by immunohistochemistry EnVision system using specific mono-clonal antibody. Results：The p27^Kip1 protein nucleus expression was low in 61 （57.55%） of the 106 cases in early breast carcinomas, which has statistic difference from the high expression in survival rate （ P 〈 0.05 ）. The p27^Kip1 protein cytosolic expression appeared in 28 （26.50%） of the 106 cases, it had worse prognosis than nucleus expression. The p27^Kip1 expression level was not correlated with age, menstruation state, pathology type, tumor size, clinical stage or hormone receptor condition （P 〉 0.05 ）, but correlated with lymph-node metastasis and histological grade （ P 〈 0. 05 ）. Conclusion ： Down-expression and cytosolic mislocalization of p27^Kip1 has been shown in early breast cancer, nucleus expression has better prognosis, p27^Kip1 may be an independent prognostic factor in early breast cancer. The expression of p27^Kip1 has related with lymph node metastasis and differentiation degree.
