异种全厚板层角膜和前板层角膜植片板层移植术后拆线时间探讨

摘　　要：目的研究全厚板层和前板层角膜移植后不同的角膜缝线拆除时间对植片的影响。方法将32只健康新西兰白兔随机分成4组,每组各8只:A组、B组(A、B2组又合为M组)为猫→兔异种全厚板层移植组。C组、D组(C、D2组又合为N组)为猫→兔异种前板层移植组。术眼为右眼,植片直径为9.0mm,植床直径为8.0mm。A、C2组拆线时间为10d(A、C2组又合为G组),B、D2组拆线时间为1个月(B、D2组又合为H组)。观察时间为3个月。结果用两因素方差分析法分析3个月时的炎性反应指数:(1)角膜植片M组(2.560±2.065)与N组(5.250±2.887)比较差异有显著统计学意义(F=15.427,P=0.001<0.05),即M组炎性反应较轻;(2)拆线时间G组(2.560±2.128)与H组(5.250±2.840)比较差异有显著统计学意义(F=15.427,P=0.001<0.05),即G组炎性反应较轻;(3)角膜植片差异联合拆线时间差异:F=7.017,P=0.013<0.05,差异有统计学意义,即角膜植片差异和拆线时间之间存在交互影响角膜移植炎性反应作用。结论角膜植片差异和拆线时间差异均存在影响异种角膜移植炎性反应的作用,并且二者存在交互作用。Objective To study the time differences of taking out corneal suture between the xenogeneic full-thickness lamellar keratoplasty and anterior lamellar keratoplasty.Methods Thirty-two health 18-week-old male New Zealand white rabbits were randomly divided into 4 groups, 8 rabbits in each group,group A and group B （group A and group B were also combined to group M） for the cat xenogeneic full-thickness lamellar keratoplasty onto rabbits;Group C and group D （group C and group D were also combined to group N） for cat xenogeneic anterior lamellar keratoplasty onto rabbits.The operated eye was right eye.The diameter of the donor graft was 9.0 mm and the diameter of receipt bed was 8.0 mm.The observation period was three months.The time taking out corneal suture of group A and group C （group A and group C were also gathered into group G） were 10 days,the time taking out corneal suture of group B and group D （the group B and group D were also combined to group H） were 1 month.Results The inflammatory reaction index differences were analyzed with two factor variance analysis after 3 months.The difference of corneal graft was significant between M（2.560±2.065） and N（5.250±2.887;F=15.427,P=0.001〈0.05）.The inflammatory reaction in group M was lighter than that of group N.The time difference of taking out corneal suture was significant between G（2.560±2.128） and H（5.250±2.840;F=15.427,P=0.001〈0.05）.The inflammatory reaction in group G was lighter than that of group H.The difference of corneal grafts and time difference of taking out corneal suture were statistical significant（F=7.017,P=0.013〈0.05）.There were interactions between the methods of corneal lamellar keratoplasty and the time of taking out corneal suture,which affected inflammatory reaction after xenogeneic lamellar keratoplasty.Conclusion The method of corneal lamellar keratoplasty and the time of taking out corneal suture both can affect inflammatory reaction after xenogeneic lamellar keratoplasty,and there are interac
