玻璃体后灌注在无囊膜支撑无晶状体眼手术中的应用

摘　　要：目的探讨玻璃体后灌注的应用对二期人工晶状体植入治疗无晶状体及玻璃体眼的影响。方法对36例(36眼)无晶状体囊膜支撑的患者施行不同手术方式的二期人工晶状体植入术,其中一术式为有玻璃体后灌注支持的睫状沟人工晶状体缝合术,另一术式为无玻璃体后灌注支持的睫状沟人工晶状体缝合术,回顾性分析视力及并发症情况。结果16例无玻璃体后灌注支持的睫状沟人工晶状体缝合术的患者术后视力0.1～0.5;有2例发生前房积血,6例继发性青光眼,15例角膜内皮水肿,16例不同程度的葡萄膜炎;而有玻璃体后灌注支持的睫状沟人工晶状体缝合术的患者术后视力0.3～0.8;仅2例角膜内皮水肿、1例继发性青光眼及20例轻度葡萄膜炎。结论玻璃体后灌注的应用对二期人工晶状体植入治疗无晶状体及玻璃体眼有较好临床应用价值。Objective To investigate the effect of vitreous posterior perfusion on the second intraocular lens （IOL） implantation in absence of lens and vitreous.Methods Thirty-six patients in absence of lens capsular support underwent the second IOL implantation,20 cases were administered IOL implantation at ciliary groove with the support of vitreous posterior perfusion,and 16 cases were administered IOL implantation at ciliary groove in absence of vitreous posterior perfusion.Visual acuity and complication were retrospectively analyzed.Results For 16 cases with IOL implantation at ciliary groove in absence of vitreous posterior perfusion,postoperative vision was 0.1 to 0.5,and there were 2 cases with hemorrhage of anterior chamber,6 cases with secondary glaucoma,15 cases with corneal endothelial edema,and 16 cases with different degrees of uveitis.But for 20 cases underwent IOL implantation at ciliary groove with the support of vitreous posterior perfusion,postoperative vision was 0.3 to 0.8,and there were only 2 cases with corneal endothelial edema,1 case with secondary glaucoma and 20 cases with slight uveitis.Conclusion Vitreous posterior perfusion is important for the second IOL implantation in treating eyes with absence of lens and vitreous.
