摘　　要：目的研究口腔扁平苔藓(OLP)患者病损局部黏膜上皮细胞中骨桥蛋白(OPN)的表达。方法经病理学检查确诊的OLP患者40例,其中17例伴轻度异常增生;充血糜烂型15例,网纹/斑块型25例。免疫组化法检测患者病损部位口腔黏膜上皮细胞OPN表达。以40名健康志愿者作为正常对照。结果OLP及伴轻度异常增生OLP病损局部黏膜上皮细胞OPN表达阳性率分别为65.4%和82.4%,与正常对照的10.0%比较,差异均有统计学意义(P<0.05)。充血糜烂型和网纹/斑块型OLP病损局部黏膜上皮细胞OPN表达阳性率分别为73.3%和60.0%,均显著高于正常对照(P<0.05),但OLP二类型之间比较差异无统计学意义(P>0.05)。结论OPN表达在OLP患者病损局部明显增加,提示OPN可能参与了OLP疾病的发生和癌变发展过程。Objective To explore the expression of osteopontin （OPN） in mucosal epithelium of oral local lesion in patients with oral lichen planus（OLP）. Methods Forty patients with pathologically-confirmed OLP （erosive OLP,n=15; reticular OLP,n=25） were investigated,among whom 17 were complicated with mild dysplasia. Mucosal epithelium of oral local lesion was examined for the expression of OPN by immunohistochemical method. Forty healthy subjects were served as normal controls. Results The positive expression rates of OPN were 65.4% and 82.4%,respectively in patients with OLP and those complicated with mild dysplasia,and both were significantly higher than that in normal controls （10.0%） （P〈0.05）. There was no significant difference in the positive expression rates of OPN between patients with erosive OLP （73.3%） and those with reticular OLP （60.0%）（P〉0.05）,while both were significantly higher than that in normal controls （P〈0.05）. Conclusion The expression of OPN is significantly increased in mucosal epithelium of oral local lesion in patients with OLP,indicating that OPN may be associated with the pathogenesis and carcinogenesis of OLP.
