改良定量主观整体评估方法评估血液透析患者营养状态的应用和分析

摘　　要：目的分析改良定量主观整体评估（MQSGA）方法与临床常用营养评估指标之间的关系，探讨MQSGA评价血液透析患者营养状况的可靠性。方法使用MQSGA表评估本院103例血液透析患者营养状况，同时测定患者血清白蛋白（Alb）、前白蛋白（PA）、转铁蛋白（TRF）、肌酐（Scr）及肱三头肌皮褶（TSF）、上臂中部肌肉周径（MAMC）、体质量指数（BMI）等，并统计分析比较MQSGA与各种营养指标之间的相关性。结果按MQSGA评分，103例血液透析患者中，轻中度营养不良71例（68．9％）；重度营养不良8例（7．8％）。MQSGA评分与患者血清Alb、PA、TRF及人体测量学指标包括TSF、MAMC、BMI之间有良好的相关性（r分别为-0．732、-0．841、-0．785、-0．812、-0．753、-0．901，P〈0．05）。结论MQSGA评分可较真实、客观、全面地反映血液透析患者的营养状态，是评估血液透析患者营养状态的新方法。Objective To evaluate the reliability of modified quantitative subjective global assessment （MQSGA） in estimating nutritional status of hemodialysis patients and analyze the relationship between MQSGA score and nutritional assessment indexes. Methods One hundred and three cases of hemodialysis patients were enrolled and their nutritional status was analyzed by MQSGA. Serum albumin （ Alb）, prealbumin （ PA ）, transferrin （ TRF ）, serum creatinine （ Set ） and triceps skin-fold （ TSF ）, mid-arm muscle circumference （ MAMC ）, body mass index （ BMI ） were measured simultaneously. The correlation between MQSGA and indexes of nutritional assessment were analyzed by Spearman. Results There were 71 cases （68.9%） with slight and middle degree of malnutrition and 8 cases （7.8%）with serious degree of malnutrition in 103 hemodialysis patients measured by MQSGA. The good correlation was found between MQSGA and Alb, PA, TRF, TSF, MAMC and BMI （r = -0.732, -0. 841, -0.785, -0. 812, -0. 753, -0. 901 respectively,all P 〈 0. 05 ）. Conclusions MQSCA score can really, objectively and overall reflect nutritional status of hemodialysis patients. MQSGA is a new method to evaluate the nutritional status of hemodialysis patients.
