摘  要:目的探讨Ⅲ期鼻咽低分化鳞癌中错配修复基因家族成员hMLH1和p53的表达及其与肿瘤临床病理特征的关系。方法应用免疫组化EnVison法检测58例Ⅲ期鼻咽低分化鳞癌标本。结果58例鳞癌组织中,hMLH1阳性率为79.3%,有颅底侵犯者为71.8%,显著低于无颅底侵犯者的94.7%(P<0.05)。hMLH1表达与患者年龄、性别和5a生存期无关。p53阳性率为65.5%,有颅底侵犯者为74.5%,显著高于无颅底侵犯者的47.4% (P<0.05)。p53表达在生存期≥5a者为31.6%,显著低于生存期<5a者的82.1%,P<0.05。结论Ⅲ期鼻咽低分化鳞癌中有颅底侵袭的患者更易出现hMLH1阴性表达和p53的阳性表达。p53阳性表达率随患者生存期的延长而降低。Ⅲ期鼻咽低分化鳞癌中hMLH1与p53的表达,可以作为判断鼻咽癌生物学行为和预后的指标。Objective To investigate the expression of hMLH1 and p53 protein at stage Ⅲ nasopharyngeal poorly differentiated squamous cell carcinoma and their correlation with clinical pathological features. Methods Immunohistocbemistry (EnVision) was applied to detect the expressions 6f hMLH1 and p.53 protein in 58 cases. Results The expression of hMLH1 protein rated 79.3%, and the expression of hMLH1 in invasive cranial base cases(71.8% )was significantly lower than the non-invasive one(94.7% ), P 〈 0.05. The relationship between the hMLH1 expression with age, gender and five-year survival were not observed. The p53 protein expression showed general positive rate of 65.5%., The positive expression rate of p53 protein in invasive cranial base cases (74.5 % ) was significantly higher than non-invasive one (47.4%), P 〈 0.05. Simultaneously, the expression of p53 in over five-year survival cases (31.6% ) was significantly lower than the below five-year survival one (82.1% ), P 〈 0.05. Conclusions In stage Ⅲ nasopharyngeal poorly differentiated squamous cell carcinoma, the negative expressing of hMLH1 protein and the positive expressing of p53 protein are most commonly appeared in the invasive cranial base patients. Declined expression of p53 protein is closely related to the prolonging of the survival time. To detect the expression of hMLH1 and p53 protein may conduce to evaluate the biological behaviors and prognosis in stage III nasopharyngeal poorly differentiated squamous cell carcinoma.
